Dear Customer, In 2009 we saw the world economy in turmoil. Many hospitals and free standing facilities severely limited their capital equipment budgets while waiting for signs of recovery. RFSuite Tracking & Verification System

CIVCOs RFSuite system provides a comprehensive tracking system for improving clinical workow. RFSuite utilizes RF technology to track patients, personnel, charts, lm jackets and patient positioning/treatment devices. RFSuite provides automatic verication and documentation of the patients daily setup as well as verifying that the correct patient is being treated, reducing the risk of setup errors. Patient Positioning & Treatment Device Verification

For the rst time, accessory verication and documentation of the patients daily setup is automated; RFSuite provides a level of above the couchtop verication never before achieved with conventional R&V systems. Before treatment, the therapist uses RFTrak to call up the patients treatment setup sheet and a listing of all RFSeries enabled positioning devices associated with the daily treatment setup appear on the monitor. Patient setup information is viewable by monitors located within the treatment vault and at the treatment console. RFTrak software assists in ensuring that the correct patient and the prescribed RF enabled patient positioning/treatment devices are present on the treatment couchtop otherwise a warning message prompts the therapist to correct the setup or override the software. The system stores a daily record of all patient setups along with every RF enabled device used. The software provides a means to generate either an electronic PDF le or a hard copy printout. All information is stored on the RFSuite central server and can be accessed at any time.

Storage Options
Two storage options, a cart and a wall mounted unit, are available. The Storage Cart is custom-designed to store and transport the Body Pro-Lok system. The shelves provide space to store accessories. The cart provides easy transportation from room to room and excellent storage space. For centers where a storage cart is not feasible, the Wall Storage Unit is an innovative solution. With specic locations for each bridge type and a shelf for the remaining accessories, this storage solution neatly organizes all components of Body Pro-Lok.

In Radiation Therapy, targeting a tumor with precision while it is constantly moving due to respiration or patient motion is not an easy task. Todays delivery technology and modalities (such as SBRT) demand a high level of accuracy and precision. Using IGRT to nd a tumor and identify critical structures is not enough your patients need to be immobilized and monitored for intrafraction motion. Respisens is an innovative device for patient motion tracking during Radiation Therapy treatments. The system consists of a miniature transmitter/receiver that measures the distance between two probes. If placed on the thorax or abdomen, Respisens tracks patient breathing and thus helps monitoring breath holds or out of threshold motion. If placed on a motionless part of the patient, it helps to verify his/her immobility. Watching patients on a monitor from several yards away is not an effective or efcient method for monitoring patient motion.

Difference Between a Couchtop and Overlay

An overlay lays on top of the cradle, existing support frame and/or spine of a CT, simulator or LINAC. For LINACS, an overlay will still result in the need to rotate the table in order to avoid the beam passing through the underlying steel support. A couchtop replaces the steel support system of the LINAC with a carbon ber support system, eliminating the need for table rotation. An indexed overlay is necessary for a CT to ensure that your positioning devices are in the correct location from room to room. These overlays are lightweight and can easily be removed between uses on a non-dedicated CT.

CIVCOs carbon ber couchtops and overlays also provide an indexing system for CIVCOs wide variety of patient positioning and xation accessories. This innovative system allows a patient positioning device to transfer accurately between couchtops and overlays and quickly lock into the same location daily on the treatment table.

How Indexing Works

Every Indexed Patient Positioning System begins with an indexed couchtop or overlay. These tops are constructed of rigid carbon ber and feature an array of indexing points running down the side of the couchtop, each spaced an identical 14cm apart or 7cm apart on the Universal Couchtop. A Lok-Bar is also provided with every couchtop or overlay. Every Lok-Bar features two pins that interface with the two holes on CIVCOs line of patient positioning and xation devices. Lok-Bars remain with the couchtop.

Indexing Points

To index a product, attach the Lok-Bar to any of the multiple indexing points running longitudinally along the edges of the couchtop or overlay. Then set the patient positioning device on the two-pins on the Lok-Bar. Indexing patient positioning and xation devices to the same indexing notches for every treatment ensures that your device is in the same location and is square with the couchtop for every treatment. This denitive repositioning of the device relative to the treatment table allows clinicians to condently tighten the tolerance window in the record-and-verify system an important enhancement. The result: increased target accuracy with increased patient throughput.

Types of Indexing
Four different types of indexing are available. The Prodigy indexing system features protruding hemisphere style indexing notches and is the most versatile system for your treatment plans. The Prodigy 2 indexing system features a smaller version of Prodigy indexing and is available only on the Universal Couchtop. The Interloc indexing system features notched style indexing points. The Exact indexing system is designed for the Varian Exact notched style indexing couchtops. PinnPoint Indexing has small indexing cylinders that protrude from the couchtop. This indexing system can be used with Aluminum Rails and PinnPoint Lok-Bars (see page 18).

The new Universal Couchtop is a strong, lightweight couchtop with low attenuation, ideal for IGRT tracking. The Universal Couchtop is also available in a dosimetrically matching CT overlay to assist in dose accuracy between planning and treatment. Contact your account representative to conrm DoseMatch technology is available for your CT Simulator. The Universal Couchtop features a metal free extension seam with four extension choices: rectangular extension, Type-S extension, Posix extension (available soon) and Varian end plate. The unique prole allows CIVCOs SBRT Body Pro-Lok bridges to attach directly to the Universal Couchtop. The Universal Couchtop is capable of being installed on linac models from Elekta, Siemens and Varian. The CT overlay is compatible with seven different models from GE, Philips, Siemens and Toshiba.

Mastercouch
For particular treatment areas, the Mastercouch provides more radiationfriendly treatment options. Standard couchtops often contain metal or less efcient carbon ber parts which attenuate the beam and cause scatter in the region of interest. The Mastercouch provides a system to move these parts out of the treatment region.

The modular Mastercouch system consists of a base to replace the existing treatment couch and is extended with a treatment module. This module extends the treatment eld to provide a treatment eld that is free from metal components. In addition, positioning accessories can be attached to the indexed positioning pivots mounted along both sides of the couch. Module options include: a standard module, head & neck modules for either the Type-S or Posix systems and stereotactic treatment module.

Standard Imaging Overlays

Carbon ber overlays securely lock onto a cradle, providing a surface consistent with treatment couchtops. They are lightweight and durable, allowing them to be easily removed or reinserted for use on nondedicated imaging machines. The attachment mechanics ensure that once the top is reinserted on the cradle, it will be level and stable throughout the imaging process. CT overlays are made of carbon ber with a foam core. They have indexing options and come with a two-pin Lok-Bar providing a system for indexing positioning devices to the couchtop. MRI overlays are made of ber glass with a foam core and have indexing options and come with a three-pin Lok-Bar prividing a system for indexing MR compatible positioning devices.

Fixator Shoulder Suppression System

The Fixator Shoulder Suppression System is designed to enhance the benets of the Type-S system. The Fixator extends off the couchtop and includes adjustable padded positioners to gently cup the patients shoulders, capturing and positioning them out of the treatment eld. Posix head and neck baseplates are designed for use with the Posicast thermoplastic masking system. Posix baseplates are available in two sizes, head-only baseplates and larger T-shaped head, neck and shoulders baseplates. Both shapes are available in a carbon ber version and an acrylic version for all cases where the low attenuating properties of carbon ber are not required. Posicast Thermoplastics
Posicast thermoplastic material has been specically developed for radiotherapy applications and provides excellent positioning and reproducibility during treatment. It does not stick to the skin or hair. If needed, Posicast adheres to itself by applying local pressure. The patented composition of this unique product results in minimal shrinkage after cooling. Posicast thermoplastic materials are available in Multiperfo and Microperfo versions depending on the selected material. Microperfo masks are preferred when small sized treatment elds are to be marked onto the mask.

Posicast has a soft polyurethane coating on both sides guaranteeing ultimate comfort in handling. It is available in Multiperfo and Microperfo.

Posicast-Plus
Posicast-Plus has two different coatings: a soft polyurethane coating on the side which is in direct contact with the patients skin and a smooth spraycoating on the outside where treatment elds are to be marked directly onto the mask. Posicast-Plus material is comfortable to the skin and offers the possibility of precise eld marking. Minimal shrinkage of the material ensures rigid and reproducible xation. CIVCO offers a wide variety of thermoplastic precuts. Rectangular plates are available allowing technologists to cut the material in any desired shape. All types are sold in sets of 10 pieces. The materials have a thickness of 2.3mm. Precuts are available with reloadable or disposable L-prole mask attachment handles.

L-profiles for Mask Attachment

By using radiotranslucent L-proles tted with durable quick fasteners, a rigid mask can be molded around the contours of the patient. With this method air gaps between the inside of the mask and the head of the patient are prevented, resulting in a very reliable, stable mask.
L-profiles set Reusable Carbon Fiber L-proles are attached to the of 10 MTSIN622510 desired precut before making a mask and remain on for the duration of treatment. Disposable L-proles come attached to a mask and are a quick, cost effective alternative to reusable carbon ber L-proles.

Uni-frame thermoplastics are one of the industrys most popular choices. They provide the rigid xation necessary for todays radiotherapy treatment techniques. A standard Uni-frame mask measures 9 x 10; extended lengths are available for additional neck xation. Posifix Supine Headrests

Special 3-D formed Posix headrests are made of low density polyethylene foam, in ve colors. The red cushion positions the head with the neck straight to prevent anterior curving of the spinal cord in the treatment eld. The black cushion is used for neck tumors and has the same curvature but is extended in the caudal direction. The yellow and white cushions enable the chin to be more or less extended to move the oral cavity and mandible out of the radiation eld, or to obtain a cranial eld border which avoids the orbit (ear and parotid treatment). The blue cushion allows the chin to be xed towards the breast.

This wedge shaped cushion supports the shoulders and upper body of the patient. AccuForm Cushions are ideal for high-precision head and neck treatments when enhanced comfort, stability and repositioning accuracy are key. This soft, clothcovered pillow is lled with tiny beads coated in water-curable resin. The Precise Bite attaches easily to a thermoplastic cast and conforms to the teeth and/or maxilla, decreasing head and neck rotation, exion and extension. The Posiboard-2 Breastboard is a complete solution for the positioning of breast patients. Made of a low density foam core covered in an ultra-thin layer of carbon ber, it allows the beam to pass through the Posiboard-2 from any angle with minimal attenuation. The Posiboard-2 can be positioned at eight angles, and accommodates several comfortable and adjustable arm support options. For additional support a breast cast can be mounted using a quick release system. When the Posiboard-2 is not in use it can hang on the wall with the supplied wall-mount plate.

The Horizon Prone Breastboard is designed to assist in treating breast patients in the prone position. The Horizon is lightweight and its ergonomic design allows for optimal patient access. Three sizes of wedges are included with the Horizon, varying in size from 15 to 25cm, to allow for a more customized t for your patient. These wedges are positioned under the contralateral breast and allow it to rotate up and out of the beams path. The slight incline of the wedges allows the ipsilateral breast to fall fully into the treatment eld. A Vac-Lok cushion (see page 53 part # MTVLWGB03C) can be utilized to help support the arms. The Thorawedge is a unique positioning device allowing the patient to incline at multiple xed angles. The system consists of two wedges (5 and 10) and a comfortable arm holder. Carbon Fiber Bellyboard

The Carbon Fiber Bellyboard has been developed in order to reduce the irradiated small bowel volume of patients undergoing treatment in the pelvic region. For treatment elds including the pubic bone towards the third lumbar vertebra, reduction of the irradiated small bowel volume up to 64% (four eld box technique) can be obtained. The Bellyboard is radiotranslucent, lightweight, rigid and indexable. For patient comfort the lower part of the Bellyboard has been adapted to support the upper legs. A soft polyethylene padding on the cranial side ensures a comfortable position for the head. The Bellyboard has two apertures: The caudal aperture (1) is for the positioning of the pubic bone The central part (2) of the device is for the compression of the small bowel in order to push the small bowel cranially towards the major aperture (3). Positioning guidelines for patient set-up on the Bellyboard: As a patient positions themselves on the Bellyboard they should start cranially and slide in the caudal direction. The pubic bone must be positioned in the caudal aperture for an optimal reduction of the small bowel volume. Fiducial markers are integrated in the bottom part of the device for patient setup verication. Clear scales on both sides of the Bellyboard allow easy verication of the longitudinal position of the patient.

The Prone Thorax Support is designed to relieve chest compression during treatments in the prone position. Vacuum Compressors
The Dual Function Vacuum Compressor provides both a vacuum and inate cycle with a convenient hand control or foot control. It is available in either 110 or 220 volt versions. A new lighter weight Dual Function Compressor will be available soon, this version has been CE marked. The Single Function Vacuum Compressor is a lower cost alternative. It provides a vacuum cycle only. To re-inate a Vac-Lok cushion, simply detach the compressor and depress the end of the cushion valve. With the MTVL1BEX the hoses can be switched to provide an inate cycle.

ACCULOC IGRT
External mark setup introduces considerable target uncertainty resulting from internal organ motion and patient position variability. ACCULOC IGRT eliminates this uncertainty using implanted gold ducials or anatomic landmarks with EPID or CR images. The 3D transformation software (ISOLOC) outputs exact couch translations for highaccuracy, veriable localization. In addition, ACCULOC has the versatility to support image pairs at any angle nonorthogonal images can accommodate larger patients, hip replacements and other challenges.

Implanted Markers
Implanted gold ducials are essential for organs that move with respect to bony anatomy, such as liver and prostate. Mouse-clicks register the markers on the initial image pair. For subsequent setups the software autodetects each marker, then registers the exact position from the center of the brightest shape. Finally the software outputs exact couch translations and coordinates for high-accuracy, veriable localization.

Anatomic Landmarks
Anatomic landmarks provide a less invasive localization option for targets that are xed with respect to bony anatomy, such as head and neck tumors. ACCULOC uses unique anatomic landmarks and the full TPS DRR (digitally reconstructed radiograph) to align the target with the treatment machine isocenter.

The Kodak 2000RT CR Plus system acquires portal localization images using computed radiography (CR). This technology has been widely used in radiology departments for more than a decade. The Kodak 2000RT CR Plus provides the high-quality digital images you need for accurate target positioning using the ACCULOC IGRT localization system. Sharp, crisp images are quickly imported into the ACCULOC localization software, clearly displaying the implanted gold ducial markers for accurate, veriable target positioning. The ACCULOC IGRT localization system provides high-accuracy localization through an internal reference system of three gold ducial markers. The ACCULOC software imports the marker images and outputs both the couch moves and nal couch coordinates necessary to position the target precisely at the isocenter.

To use the Kodak System:

1. Replace the lm in a therapy or simulation cassette with a reusable phosphor plate. Then expose the plate using standard techniques just as for a lm. 2. Remove the exposed storage plate from the cassette and place in the plate reader. 3. The reader scans a 2.5 MB image in 25 seconds and produces a high-quality digital image on the display. The plate is automatically erased for reinsertion into the cassette and reuse. 4. Use the digital enhancement and processing tools to review and manipulate the images to a desired look. The images can be sent to any DICOM destination on a network, including printers, image management workstations and image archives.

Fiducial Markers | Localization

Implantable Fiducial Markers

Implantable ducial markers provide a highly effective method of ensuring accurate target localization for tumors/ organs that move in respect to external anatomy, as well as for the precision localization needed for stereotactic radiotherapy. Our IGRT program led the industry in 1989 with the rst commercially available system for cranial target localization. This momentum in research and development continues to keep us on the leading edge of advanced tumor targeting. The result is a comprehensive ducial offering.

Bone Markers
Bone Markers are pure gold 2mm spheres used in cranial and spinal applications. They are typically placed in a small hole in the bone and sealed with bone wax; with no marker migration.

Gold Soft Tissue Markers

All Gold Soft Tissue Markers are pure gold cylinders specially knurled to inhibit migration. Standard Gold Soft Tissue Markers are 3mm long and 0.8, 0.9, 1.2 or 1.6mm in diameter. They are available individually or in sterilized preloaded needles. Align Markers are 10mm long to accommodate localization software systems that utilize linear ducials as opposed to multiple segment points. They are 1.2mm in diameter. CyberMark Markers were developed specically for use with Accurays CyberKnife robotic radiosurgery system. The gold marker measures 5.0mm long and 1mm in diameter, as specied in the CyberKnife software. Coupled Markers are 3mm long with either 0.8 or 1.0mm diameters attached at a xed distance 20mm apart from center to center. They are secured with a sterile, tissue-absorbable material. This ensures that the markers are implanted at a known distance for calculating precise localization.

Carbon Soft Tissue Markers

CIVCO continues to optimize imaging solutions for new IGRT modalities such as kV and cone-beam CT. The new carbon markers provide high-resolution kV localization requiring less dense materials with minimal CT artifacts. Carbon markers are ideal for a wide variety of imaging modalities, including x-ray, mammography, uoroscopy, kV, CT, MRI and ultrasound. The unique shape of the carbon markers inhibits migration.

Suture-type Markers
Suture-type markers are designed for high accuracy breast tumor localization. They are applicable in any physician-designated location where migration potential is a concern. The pure gold spheres with small central holes are readily sutured in place to provide a permanent localization solution. Suture-type markers are available ready for placement prethreaded on absorbable sutures.

Merrield Cartridge
The Merrield Cartridge allows you to deploy two or more markers through the same needle with only one needle stick. After deploying the rst marker, the stylet is simply removed and the Merrield Cartridge is easily attached to the end of the needle. The tip of needle is then repositioned and the next marker is deployed. Depositing multiple markers with only one needle can be advantageous in a variety of different body locations. Patient comfort is increased by minimizing needle sticks.

Surgical Tool Kit

An EPID Based 3D Absolute Patient Dosimetry System
Why perform 2D single plane IMRT QA for 3D complex treatments? Presently all QA analysis is calculated on a single plane not in or on the patient volume. The assumption being that acceptable measured results on a single plane equate to an approved treatment for target volume and critical structures. Experience has now shown that this is not necessarily true. Dosimetry Check works with any EPID and any treatment planning system to provide a volumetric measurement for IMRT QA. Unlike other systems which use single plane QA Analysis, Dosimetry Check uses the patient CT data set as a Phantom. This is done by converting each beams 2D image information to a Measured Calibrated Fluence which is then projected onto the patients CT data set. In this way the delivered dose is reconstructed with the same CT data set as the original treatment plan thus allowing for planned and measured dose comparison. Plan comparison includes isodose overlays, dose proles, point doses, dose differentials, 3D surface doses, 3D gamma analysis, Gamma Volume Histogram (GVH) analysis, and Dose Volume Histogram (DVH) analysis. This water-equivalent phantom material eliminates mass and maintains your ability to perform routine quality control checks of your linear accelerator. Whether using electron or photon beams, Virtual Water meets your needs for every application. Virtual Water is free of contaminants and air. Each production run of Virtual Water is tested in an independent calibration lab certied to be within 1% of water at photon energies. Virtual Water is available in 20 x 20cm, 30 x 30cm and 40 x 40cm sizes and in thicknesses that range from 0.1 to 6.0cm. Cavities are available in thicknesses of 2.0cm or greater to accommodate many commercially available ionization chambers. Virtual Waters rigid design eliminates the potential for chamber damage. Water Tanks
CIVCO offers two compact water tanks that are 30.5 x 38 x 38cm or 38 x 38 x 38cm with 9.525mm thick acrylic walls. They also include a horizontal scale and water drain kit, as well as the choice of either a manual depth dose apparatus or a motorized depth dose system with remote control/readout. The water tanks are small enough for convenient, quick setup; yet provide a sufciently large internal volume for analysis of clinically relevant areas of radiation therapy beams. These water tanks are compatible with TG51 and other protocols for beam quality control.

Manual Depth Dose System (shown above)

This device is constructed from acrylic, brass, teon and stainless steel for accuracy and durability. The probe drive attaches to the side of the water tank and a top-mounted manual crank and a four-digit counter are provided for exact vertical positioning of the measuring probe within the tank. The total vertical travel of the probe holder is 25cm. The mechanical resolution of the vertical drive mechanism is .1mm. The manual depth dose device may also be relocated horizontally along the wall of the tank using the horizontal scale provided. Acrylic probe holders for micro chambers, farmer-type chambers or parallel plate chambers are included with this system.

The Motorized Depth Dose System employs all the mechanical components of the Manual Depth Dose System with the added convenience of controlling the vertical probe position remotely from the console. A motor controller/readout module with control switches and four-digit LCD counter enable the physician to remotely drive the ionization chamber within the tank. The motorized depth dose apparatus eliminates the tedium of repeated manual repositioning of the probe. It is available in 110 volt only. An upgrade is available for those who already have the Manual Depth Dose System.

The 0.6cm3 PTW Farmer chambers are designed for absolute photon and electron dosimetry with therapy dosemeters. The type30013 is the waterproof standard chamber for absolute photon and electron dosimetry. The nominal useful energy range is from 30 keV to 50MeV. This chamber type is of rugged construction, since the wall material is graphite with a protective acrylic cover and the electrode is made of aluminum. The guard rings of all chamber types border the measuring volume. An acrylic build-up cap for in-air measurement in Co-60 beams is included (chambers 30010/11/12), as well as a calibration certicate.

Styroformer
Three Styroformer units are available to meet your foam cutting needs. The Styroformer is only available in 110 volt versions but a 220 volt upgrade is available. The Basic Styroformer includes a verication light, three tracing tips (20, 40 and 60cm) and self-locating block holding arms. The Styroformer Booster System includes all the features of the Basic Styroformer with a hot-wire temperature booster and alarm system. The booster system increases cutting wire temperature when the wire deects a given amount. A simple switching system reroutes the current through another circuit with its own potentiometer, which the operator can set to his/her preference. The Styroformer Powerlift Light has all the features of the Basic Styroformer but also includes motorized adjustment of source and block arms.

Foam Cutter
This countertop foam cutter features an easyto-change NiChrome wire assembly and a large cutting surface, making this an ideal unit for cutting electron foam. The Foam Cutter is available in both 110 or 220 volt models.

Compucutter
The Compucutter uses precise, digitized radiograph outlines to cut shielding block molds with 1mm accuracy. Block outlines can be manually digitized or transferred from treatment planning systems. Air Sampling
To comply with OSHA regulations, every facility that fabricates shielding blocks should conduct air sample analysis and post the results. CIVCO offers one-week rentals on OSHA-approved air sample pumps and lters. Simply conduct an eight-hour sample according to the instructions, then ship the pump and filter back to CIVCO. A written analysis will be sent back in three to four weeks.

Wipe Test
The convenient mold room wipe test includes wipes, sample containers and a test area template. Simply conduct the test according to the instructions and return the sample to CIVCO. Tungsten eye shields have been clinically tested and proven to provide adequate protection of the ocular structure for electrons up to 9MeV. Each eye shield is coated with dental acrylic on the beam entrance side of the shield to reduce electron back scatter to an acceptable level. Eye shields are sold separately in three sizes: small (19.5mm), medium (23mm) and large (25mm).

Testicle Shields
Testicle shields are designed to reduce scatter radiation to the testicles. Each unit is cast to provide a lead wall with a thickness of (1.25cm) and features an open sector to allow comfortable attachment to the patient. All surfaces are coated with a polyurethane paint to reduce lead exposure. The shields top and bottom sections are secured with either the two included rubber straps or nylon tape. When using the testicle shield stand, no straps are needed a special dome and groove between the two halves of the shield stop leakage and prevent them from sliding apart.

Bolus
Bolus is made from a solid, homogeneous, tissue-equivalent gel with a density of 1.03g/cc. It will not sag, deform or run. Bolus is available encased in a tough layer of thin plastic skin, or skinless or sticky for greater conformity to steep body contours. It is closely controlled for uniformity of thickness, electron density and homogeneity. Bolus sheets can be cut with a scissors to any desired shape and layered to obtain the desired bolus effect. Bolus is the ideal choice for applications above 1MV where additional tissue-equivalent thickness is required. It can also be used as a phantom material due to its homogeneous composition and uniform thickness.

Calipers
Body Calipers can be used for tabletop distances to determine central-axis and off-axis measurements. The reversing caliper orientation assists in determining separations such as laterals, obliques and off-table measurements. The wide base ensures perpendicularity to tabletop. A color-coded scale on both sides eliminates confusion with scale orientation. It is made of precision-machined aluminum with a gold-tone nish. Components are hand-tapped, counterbored and assembled with steel screws for strength and stability. Payment Terms Net 30 days for customers with approved credit inside the United States and Canada. All orders must be paid in U.S. dollars. Orders from outside the U.S. require advance remittance by wire transfer. Visa, Mastercard, Discover and American Express are accepted. Alternate International payment terms are considered upon request. Delinquent accounts are subject to suspension until payment arrangements are made. Shipping Terms F.O.B. point of origin. Freight is prepaid and added to the invoice. CIVCO will select the most efficient method of shipment, unless a specic carrier is requested. Overnight and rush shipments are provided for an additional fee. Pricing and Discounts Prices are subject to change without notice. Quantity discounts and blanket order pricing are available. Contact a customer service specialist for details. A $50 minimum purchase is required with each order. Sample Requests Free non-sterile samples are available on selected products. Samples are provided for sizing and t purposes only and should not be used in clinical procedures. Return Policy If you are dissatisfied with a product, contact a CIVCO customer service specialist for advance return authorization and instructions. Returns should be arranged within 30 days of the invoice date and are subject to a 25 percent restocking fee. Returned goods must be in original packaging and in new condition. CIVCO reserves the right to refuse unauthorized returns. Claims, Shortages or Damages Please open all shipments immediately and inspect for concealed damages, shortages or errors. If goods were damaged in shipping, please hold the package and request a carrier inspection and claim form. Please report any shortages or errors to your CIVCO customer service specialist within 30 days of receipt of shipment to arrange an authorized exchange. Product Warranty Reusable products. CIVCO warrants products manufactured by CIVCO and intended to be reusable shall be free from defects in materials and workmanship for 12 months from the date of invoice by CIVCO unless otherwise stated on the product labeling and/or instructions. Disposable products. CIVCO warrants products manufactured by CIVCO and intended to be disposable (single-use) shall be free from defects in materials and workmanship for 12 months from the date of invoice by CIVCO unless otherwise stated on the product labeling and/or instructions. Sterile disposable products shall be sterile for not less than 12 months from the date of invoice by CIVCO unless otherwise stated on the product labeling and/or instructions. All products not manufactured by CIVCO are warranted if applicable as dened under the original manufacturers warranty as stated on the labeling and/or instructions. Buyers sole remedy for breach of this warranty will be replacement or repair of the product or part, at CIVCOs option. To the full extent allowed by law, the foregoing warranties and remedies are exclusive and in lieu of all other warranties, terms, or conditions, express or implied, either in fact or by operation of law, statutory or otherwise, including warranties, terms, or conditions of merchantability, tness for a particular purpose, satisfactory quality, correspondence with description, and noninfringement, all of which are hereby expressly disclaimed. Notwithstanding the foregoing, CIVCO makes no warranty, nor shall it have any obligation to the customer with respect to any product sold hereunder, if such product has expired according to product labels or has not been used, handled or stored in accordance with written CIVCO guidelines. Disclaimer. Some countries, states, or provinces do not allow the exclusion or limitation of implied warranties or the limitation of incidental or consequential damages for certain products supplied to consumers, or the limitations of liability for personal injury, so the above limitations and exclusions may be limited in their application to buyer. When the implied warranties are not allowed to be excluded in their entirety, they will be limited to the duration of the applicable written warranty.
