16, 2014;
That he is a member of the Social Security System with ID number
03-7215411-8;
That the service of a funeral parlor was not contracted;
That the funeral expenses amounted to more than Thirty Thousand
Pesos (Php 30,000.00), some of the receipts hereto attached, while
some were not documented;
That I am executing this Affidavit to attest to the truth of the foregoing
facts and to use the same for whatever legal purpose it may serve.
