CYT DuPage Backstage/Crew Sign-Up

Dress Rehearsals:
Mon – Wed, Feb. 22-24, call: 5 - 9pm

School Shows:
Thursday & Friday, Feb. 25 & 26,
call: 8:30am-3pm, shows: 9:30 & 11:45am

Public Shows:
Thursday, Feb 25th call: 6-9:30pm, show: 7pm
Friday, Feb. 26th, call: 6-:9:30pm, show: 7pm
Saturday, Feb. 27th, call: 1-8:30pm, shows at 2 & 6 pm
Sunday, February 28th, call: 1pm until done moving out! Shows: 2 & 6 pm

______Program Passer Will be under an adult supervisor. Please indicate which performance(s) you are available to pass out
programs to the patrons coming to the show.

13-18+ other Behind the Scenes


_____Back Stage Crew (is a great way to be a part of behind the scenes.) Must be willing to work hard moving sets & props
as directed during the show. Must commit to ALL dress rehearsals/performances and follow directions well. Seeing one
performance from the audience will be at the chairperson’s discretion.

____Ushers (help patrons to their seats, light walkways for patrons who get up during the show & possibly for the cast.
Must be 18 or at the discretion of the House Chair.)

_____Props (help collecting and setting props)

_____Make-up (must have experience and approved by Make-Up Chairperson)

_____Souvenirs (selling show items in lobby before, intermission, and after shows)

_____Costumes (must be able to do simple sewing)

_____*Tech Crew (help with mics, lights/spots, sound, assisting Tech Chairperson)
_____* Sets (help paint or work on set construction at CYT Warehouse in Cary, IL)

*Must be at least 14 years old or at discretion of Tech/Sets chair

Authorization and Release

The undersigned, the parent or legal guardian of the above-named minor, grants permission for the
above-named minor to participate in classes, camps and other activities provided or sponsored by CYT
Chicago, Inc. ("the CYT activities"). The undersigned understands and acknowledges that the above-
named minor may suffer injury, sickness or other physical harm from participating in such activities. The
undersigned further understands that the undersigned is responsible for all costs and charges for medical
treatment that may result from such injury or sickness, or other damages that otherwise result, relate to or
arise from participation in the CYT activities.

The undersigned further understands and acknowledge that CYT Chicago, Inc. will not allow the
undersigned or the above-named minor or immediate relatives thereof to participate in the CYT activities
without releasing and holding CYT Chicago, Inc. harmless. The undersigned requests that CYT Chicago,
Inc. allow above-named minor to participate in the CYT activities, and in consideration thereof, agrees to
release, and forever discharge CYT Chicago, Inc., its officers and directors, and their employees, agents
and parties volunteering on behalf of CYT Chicago Inc. from all actions, causes of action or claims for
damage of any kind growing out of or relating to any of the CYT activities in which the undersigned or
above-named minor or their immediate families participate. The undersigned acknowledges that this is a
full and complete release of all injuries, sicknesses and damages that the undersigned or above-named
minor or their immediate families may sustain or incur as a result of participation in the CYT activities.

The undersigned gives permission to CYT to use any and all photographic portraits or pictures, television
spots, movie films, videotapes, web sites and/or sound recordings, or any part thereof, taken or created
during the CYT activities which may include the undersigned or above-named minor or their immediate
families in promotional materials.
