Name: Cornelius Fudge (Handle: CornFudge321) So, this is what you need to
do. Put in your FULL name, if you feel comfortable doing that on the web. If not,
just put your Jedi Handle (the name that appears as your username on the
Institute forums). If you are putting your full name, include your handle in
parenthesis, as they could be completely different.
Advisor: RiddleNox (Whoever reviewed your PLP. If you ONLY need this form, put
in RiddleNox like shown. I put this tutorial together)
Date: June 11, 2011 (Please put the date down with the MONTH written out, the
date, then a comma, then the year fully written out. This is American style and
easiest for the staff to decipher on quick reference if need be. In addition,
please write the DATE YOU FINISHED THE PLP. If it takes you more than one
day, only put the day you finished it.)
Rank working toward: Novice Rank (You are working toward Novice Rank. So,
this is already filled in for you. When you start working toward Adept, just
change the wording.)
Integrative Practice Hours
(Ok. So, this is where it gets tricky. You are going to use pages 37-38 of the
Introductory Course packet to fill this section out. You may only need 5
Activities, but I am going to show you a PERSONALIZED example of how this
section works. First off: choose activities that interest you. You dont HAVE to go
to a book club if you dread reading.) All of this information is in the
INTRODUCTORY course packet on pages 37-38. Please READ before asking
questions, and follow this example. But, use your own activities and hours.

Number of Hours:
rank is also acceptable. If you are unsure about your poor, ask the faculty
member reviewing your PLP.
You need evidence through journaling (at a minimum) for: YOGA, TAI CHI, QI
GONG, PILATES, MEDITATION CLASSES, MARTIAL ARTS CLASSES.
Activity 2: Gym Membership August 10, 2012
10 hours.
You do not need an instructors signature. But, you DO need to keep a journal
(you can post on your blog or preferably on-site training journal) that dictates
your progress and general attendance. If you are trying to gain muscle or lose
weight, think about shooting BEFORE and AFTER pictures. This will show the
staff your progress. This is optional.
Activity 3: Volunteer Work
November 5, 2012
10 hours.
For volunteer work, you need to keep a LOG of the hours you volunteered, and
some contact details of the organization you served under. We need these
contact info to check references and authenticity if needed.
For Offline courses, please JOURNAL about your experience. Remember, the
Integrative hours are meant to help you GROW in your Jedi training. Tell us your
benefits. Tell us what you didnt like. Whatever you think we should know!
Activity 4: Book Reports
December 10, 2012
10 hours(4 books)
For Book and Movie Reports, write a 500 word book report and post it on the
forum in the book review area. Easy enough. Simple enough. I think this is the
most important one! Fill up these 10 hours! Include links here to reports on the
forum.
Activity 5: Student Council
President
June 9, 2012
10 hours
March 4, 2012
7 hours(7x)
Acupuncture healing is NOT on the list! But, dont you think it fits under the
healing treatment category? Go ahead and add things to what is suggested in
the Intro course packet pgs. 37-38 if you feel like they belong, and then verify
with Jax. Just make them part of a category and do not exceed the maximum
hours of a single category. Of course, healings have NO limit, so it is ok to have
10 total hours of healing like it says in the example.
Total Hours = 60.
You should total up to 50 hours at LEAST. If you have more, then put more. That
is why we have the total hours listed. Any hours over can be counted toward
your NEXT level status. But, for now, list them all, even if it is just 10 hours
over. Thank you!
Institute Courses
For ALL students you must first take the Intro course. You have already
completed the intro course if you are filling out this PLP. So, please place the
date you completed the introductory course in the format we request as shown.
In addition, all students will take Creed 101 and Jedi Studies 101 as their first
courses. These are listed already on your PLP. As you COMPLETE these courses,
you will need to ADD courses to this PLP and put your completion dates. That
means that as you have this PLP, you will be UPDATING it as you need to until
you FINISH your goals or change them due to your needs and interests. When
you are done with the Novice training, you will be presenting this document in
your Novice Trials, so update as you go.
Course: Intro Course.
Give yourself at least 2 months from the end of your intro course to complete
one course. Give an extra month to the other. This will give you PLENTY of time
to focus and fit these courses in when you can. They are self-paced, so do not
think you need to finish them immediately.
Additional Goals:
CPR Certification: _____None____ if none:
Date of Completion: November 3, 2012
First Aid Certification: ____None_____ if none:
Date of Completion: November 3, 2012
For these two above, make sure you get SOME kind of form or license that says
you have these certifications. Have them scanned (or take a picture) and email
them in to the Institute. When you are ready for that, contact Jax or Connor for
email addresses. These are required! If you need help signing up or finding
places in your area, contact the same people and we will help you find some
courses in your area for free or that are inexpensive.
Personal Goals: Become a College Graduate with a 3.5 GPA or higher.
Maintain my integrity in school.
Do 365 good deeds this year.
Meditate everyday.
Put at least 4 good goals in list format here. Make them simple, attainable, yet
challenging! You will use these in the next heading.
Obstacles you face in achieving those Goals: My lack of motivation.
My need for an easy way out.
My apathy towards the world and the community.
My busy schedule.
If you look, each of my obstacles corresponds to the list above in the personal
goals. Make them general issues you have. Dont put so specific issues that
they are able to be overcome easily. Usually a broad spectrum of issues is a
life-long challenge. And, this is ok!
Solutions to those Obstacles: Keep a study planner. Set times for when I can do
what I need to do.
Be smart by doing my own work. Even if it hurts my average, I will at least
KNOW what I have learned and can assess my issues from a less abstract
perception and tackle my issues with a clear focus.
I should make myself do good deeds every day. This is what being a Jedi is
about. Helping others.
So, there you have it!
We, at the Institute, understand your busy schedule. We definitely have those
as well. If you start your PLP journey, and half way through you realize you
have set unrealistic goals. CHANGE THEM! Be smart! Be flexible. Nothing that is
written here cant be changed. You can keep editing for months if you want to.
Just know that you should hold yourself to realistic goals. That is why you
submit and publish your PLP when you are finished. That way, the staff can
gauge from experience whether or not you have allowed yourself enough time.
