APPLICATION CHECKLIST
List of Documents for Submission
EACH application set MUST consist of the following:
(Please tick for documents submitted)
Completed Acknowledgement Slip, affixed with Singapore stamp of 50 cents for Singapore addresses
only (click on the link to download)
Bank draft or cheque (S$20 for each M.Sc. programme applied online) to be made payable to National
University of Singapore / transaction receipt for online payment of application fee
Completed Application Fee Form (click on the link to download)
Completed and signed Summary Form printed for your online application (To obtain this Summary Form,
click on Manage Applications at the menu, and then click on the Application Number of your online
application for the relevant programme.)
Completed Objective of Pursuing Course (click on the link to download)
Supporting Documents (photocopied in A4 size and to be arranged in the following order)
Passport page bearing the passport number and other particulars (for non-Singaporeans)
Singapore NRIC or citizenship certificate (for Singaporeans)
Entry permit (for Singapore Permanent Residents)
Employment Pass / Work Permit (for International applicants working in Singapore)
Documentary proof of change in name (if you had your name changed and your name on NRIC /
Passport is not identical to the name printed on your degree scroll / certificate / university transcript)
Polytechnic results and certificate or its equivalent (if applicable)
Photocopy of University Transcript / Results Slips (from 1st year to final / Honours year, including
explanatory notes on grading system) to be submitted at the point of application
(Note: The original / certified true copy of the transcript for candidates with qualifications other than
those from NUS would be requested at the point when the offer of admission is made. This must be
sent DIRECTLY from the issuing University to NUS, enclosed in an official SEALED envelope with its
flap bearing the security seal of the University and the signature of the Registrar or representative.)
Degree scroll / Degree certificate (if you are a current student, state the expected date of completion of
degree: ____________________)
Financial statement indicating a balance of at least S$18,000 or its equivalent (for International
applicants applying for full-time courses)
Scoresheet indicating a minimum TOEFL score of 600 (paper-based test) or 250 (computer-based test)
or 92 (internet-based test) OR a minimum IELTS (Academic) score of 6.5. Only the internet-based
TOEFL and IELTS (Academic) scores are accepted for NUS-TUD DDP.
GRE scoresheet (optional)
Referee reports / recommendation letters (optional)
Note: All supporting documents submitted in a language other than English MUST be accompanied by an
officially translated and certified copy in the English language. Documents submitted should be A4-sized and
all envelopes are to be removed. Original copies of supporting documents submitted by candidates will NOT
be returned.

For each application set, please help us to arrange your documents in the following order (with all the
envelopes for supporting documents removed) as shown below diagrammatically. Please do NOT file or
bind your application set.

IMPORTANT:
You are required to submit an application set for EVERY M.Sc. programme that you have applied online.
For example, if you have applied for THREE M.Sc. programmes online, you are required to submit THREE
application sets in total. They can be sent together in one package.

Submission of Application Set(s)


Please send (either by hand or by post) the physical copy of the application set(s) to:

If sent by post, the use of courier service/registered mail is strongly encouraged.
Please indicate the programme(s) applied, in order of preference, on the bottom left corner of your package.
If you are sending your application set(s) via courier service, please state this information in the Description
Section of your Consignment Note.
Note: We regret that request for change(s) to the application(s) submitted will NOT be entertained.

IMPORTANT:
Your application for the M.Sc. programme(s) will ONLY be processed when you have completed the online
application AND send the physical copy of the application set(s) to us not later than 27 August 2014.
Extension of deadline is not allowed.
Therefore, you are strongly advised to plan your schedule ahead so that you could complete the whole
application process [i.e. apply online and send out the application set(s)] by the deadline.
Note: We regret that any online application WITHOUT the submission of physical application set will be
considered as INCOMPLETE and will NOT be processed.
