COMMITMENT FORM

This Commitment Form (the Agreement) is made and effective on August 25, 2008,
BETWEEN:

FAIRCHILD CEBU COMMUNITY CREDIT COOPERATIVE (the "Firm"), a


corporation organized and existing under the laws of the Philippines, with its
head office located at: Fairchild Coop Bldg., 2 nd street, 2nd Ave., Mactan
Economic Zone 1, Lapu-lapu city, Cebu Philippines 6000

WHEREAS, the parties agree to certain terms on sales and services to be performed by the Firm for the
Client; (Please check the following that applies)

Globe Visibility sub-dealership


Globe Ipalm Top-up Machine
Globe IAMAX Instant Load
Globe AMAX Phone

THEREFORE, the parties agree as follows:


1. TERMS
This commitment form is to serve as Client's written authorization for Firm to perform sales and services
and/or contract with suppliers for the above described items or services. Client's signature also indicates
understanding of estimates, prices, terms and liability to Firm for said items or services. Alterations or
revisions of above specifications involving extra costs will be executed only upon additional written orders.
2. DESCRIPTION OF THE SERVICES
The following are to be performed by the Firm for the Client (Please check all services that applies);
Globe Visibility Sub-Dealership

Commitment Form

Globe Visibility is a prepaid mobile internet USB Device


Price per unit is Php 3,800.00
Minimum order value is 1 Box per month (equivalent to 50 pcs)
Incentives of sub-dealers will be as follows;
Orders of 50-75 units/month on a single receipt will be Php 100.00
Orders of 75-100 units/month on a single receipt will be Php 120.00
Orders of 100-150 units/month on a single receipt will be Php 130.00
Orders of more than 150 units/month on a single receipt will be Php
150.00

Globe Ipalm Top-up Machine

Globe Ipalm Machine is an automated loading machine intended for self-loading


Price per unit is Php 40,000.00
There will be no minimum order value
The Client will have Php 5,000.00 rebate as incentive of the purchase/resell of
the machine
Price already includes setup of the machine and initial training
Price excludes shipment and delivery of the machine, for training request outside
Cebuthe client will shoulder the transportation, accommodation and meal of
the company representative

Globe IAMAX Instant Load

Globe IAMAX is a system based auto-loading for the clients members


Minimum load order value is Php 20,000.00 per month
10% off the load value will be the incentive of the client
Setup and Initial training will be FREE
The client will have to shoulder transportation and accommodation of trainer for
setup requests outside CEBU

Globe AMAX Phonekits

Globe AMAX CE is a program where everyone can purchase the phone even a
lot cheaper than the acquisition price of each phone
The Client will have the liberty to price the phones according to their profitability
standard
Prices and Models of the phones included on this program are as follows;

Phone Price will only be Php 800.00
Monthyl Load requirements Php 500.00 for 24 months
Client or Reseller will enjoy 10% off from the Load value
as income

Phone Price will only be Php 1,000.00
Monthly Load requirements Php 800.00
Client or Reseller will enjoy 10% off from the Load value

3. PAYMENT TERMS
All payments should be in cheque addressed to FAIRCHILD CEBU COMMUNITY CREDIT
COOPERATIVE. Globe Visibility Device, Globe Ipalm Top-up machines and Globe IAMAX Load shall
be paid COD. Globe AMAX Phonekits payment shall require post dated checks for the entire 24
month load values.
IN WITNESS WHEREOF, the parties hereto have executed this Agreement on August 25, 2008.
