BUSSINESS OVERVIEW
Ultimate love story is a name of our company which is created by a group of 5 member as a
shareholders. We begin our business on October 10th, 2011 in a small office located at Jalan D
9/D, Seksyen 9, Shah Alam, Selangor. Our business is a wedding planner where we help couples
have the wedding they want. This could mean planning and organizing the whole wedding from
start to finish or just sorting out certain aspects of the day.
VISSION
In the future, Ultimate Love Story wedding planner will be recognized as an elegance and
sophisticated wedding planner that give experience that will leave you glowing both inside and
out.
MISSION
We take pride in our work, therefore with our years of experience, qualified staff, excellent
coordination and planning skills, we guarantee you 100% satisfaction when you allow us to take
charge of every detail. We hope to be able to service you, when we do, we will make sure that in
that special day you and your guest can enjoy a grand event leaving with nothing to do except

Regulations on Customers
1. Pre wedding Blood Test (Thalassemia)
It is to check whether both carry any genetic disease which can effect theirs future
generations.
The chance of the baby getting thalassemia disease is higher when both parents have
that disorder. If one does not have then it might or might not have this disorder.
2. HIV Test
Muslims also have to undergo HIV blood test although it is up to the partners to agree
whether they want to continue with the marriage in the event their spouse fails the test.
3. Wedding Vow
Some marriage person cannot take out their ring because of their belief and marriage
vow to their partners.
However, based on safety, it is legal for them to remove the ring based on the
situations. Meaning that, when the person deal with the situation that can cause:
a) Hand get caught in the machinery
b) Rip finger off
c) Lose stones in the food
d) Prongs could harbor bacteria
e) Contaminate the product
The person who own the wedding need to inform the police about their parking facilities
so that public safety and obstruction in area can be control.
Parking facilities means that area has been vented by police and has measure in place
to create safe environment.
Regulation on Workers
1. Worker Compensation Insurance
Company responsible for the cost of their workers medical bills if the workers are
injured while carrying out their duties.
Can help offset these cost by covering expenses related to the workers on the job
injuries or illness such as:
a) Medical expenses including immediate medical attention, surgery and
treatment.
b) Lost wages while employees recover from a work related injury or illness.
c) Court costs if the employees sue your business for their injury.
d) Support payments to dependents and funeral costs when an employee dies
from their job related accident or illness.
2. IDENTIFICATION OF HAZARDS
As part of any good planning process hazards should be identified and risks assessed and
controlled to minimize the potential for injury or harm. Events vary in size, nature and type, but all
events require assessment, control and monitoring of risks. Hazard identification is a process
used to identify possible situations where people may be exposed to injury, illness or disease, the
type of injury or illness that may result from these and the way in which work is organized and
managed.
In order to satisfy certain themes like traditional, modern or garden theme, people tend
to use a lot of lights. Too many lights use high voltage that may lead to fire.
In the wedding day, a lot of electrical tool are used but there also have a lot of food
and drink. Water is a good electric conductor and if the electrical tool not proper wiring
and some water spill on it, it might cause electrical shock to someone accidently touch
it and it might cause serious burn to the victim.
For technician that we give responsibility to handle the something that related to
electrical tool, if they not properly wear a suitable clothes and tool, it might cause
danger to them because watches, ring, neck chain or other current-conducting apparel
can conduct the electric very well.
2. Health Hazard
Health and safety hazards exist in every workplace. Some are easily identified and corrected,
while others create extremely dangerous situations that could be a threat to your life or long-term
health. The best way to protect yourself is to learn to recognize and prevent hazards in your
workplace. Some of the health hazard that might occur in wedding day re:
In some wedding, the volume of the music maybe too high that can cause damage to
eardrum.
Decorations that are not recyclable can cause pollution if there is not proper disposal.
If catered food gone bad, it may cause food poisoning to the guests and hosts.
3. External Threats
External threats events that occur in the community outside the hospital that may effect the
facilitys ability to carry out regular activities. Examples of external threat are hurricanes, tornados,
floods, storms, earthquakes, train derailments, plane crashes, power outages, terrorist attacks,
and riots.
If it raining too heavily with lighting and thunder, it may strike electrical circuit and
cause fire afterwards. Hence, it poses danger to the attendees and environment.

4. Biological Hazard
Biological hazards are organic substances that pose a threat to the health of humans and other
living organisms. Biological hazards include pathogenic micro-organisms, viruses, toxins, spores,
fungi and bio-active substances. Biological hazards pose the greatest immediate food safety
threat to the consumer. For example, the capacity of food-poisoning bacteria to cause large
outbreaks of acute illness is an ever-present threat in the food supply chain. It is microorganisms
and food-borne parasites that are of most concern as biological food safety hazards.
If the food with poor sanitation, a wide range of intestinal parasites can be transmitted
to humans via contaminated foods.
Viral gastroenteritis is very common worldwide and there are a number of viruses that
are capable of causing food-borne infections. The best known are noroviruses and
hepatitis A, which have been responsible for serious food-borne disease outbreaks

5 Other hazard
Some clients want their wedding seem to be more romantic so they usually used
candle in order to set the mood. In wedding, in guest list, it not only has grown people
but children also include in that list. As we know, children like to play around with their
friend and it might crash to the table and beautiful setting candle on the table might fall
and then cause fire.

4. WASTE MANAGEMENT
During marriage season, between 50 kg and 500 kg of garbage is collected in a day. As a first
step towards ensuring a garbage-free zone, it is crucial to serve notices to all marriage halls and
laid down a set of guidelines for them to manage waste and garbage scientifically.
Use electronic invitations that is common and acceptable to save the date for friends and
family.
Halls have been told to put adequate bins or containers inside and outside the halls to
encourage people to throw waste into bins rather than litter the place.
The owners also have to inform the garbage contractors of their respective areas to
dispose waste in a proper way. Those failing to comply may be penalized.
Wedding halls are advised not to use plastic cups or plastic bags and to restrain people
from littering the hall and its vicinity.
Use home-made candles, local river rocks, edible favors, reusable ribbon, fruit,
evergreens for creative and green decoration
Dont use plastic flowers for decorations, instead use fresh flowers that are in season.
Dont send food waste to landfill sites, instead accumulate all food wastes to make a
natural DIY compost.
A dedicated Fire Control Center monitors the Fire Panel 24 hours a day, 7 days a week.
Sprinkler system throughout entire Hotel (guest rooms, hallways, meeting rooms, Grand
Ballroom, public areas, back of the house).
Hard wired smoke and heat detectors throughout entire Hotel (hallways, meeting rooms,
spa, kitchens, public areas).
Dedicated and marked Hard Standings for Fire Brigade with easy access to Breaching
Valves.
Fire alarms, visual fire alarms (strobes) and audio (whoop siren) located throughout entire
building.
Fire dampers on each floor are designed to close when smoke detectors are activated.
Evacuation
Walkways and signs are well lit and free from obstacles.
The hotel utilizes a Public Address (PA) System to communicate to all areas of hotel in an
emergency.
Evacuation drills are conducted every three months with hotel staff.
Evacuation procedures are in place for assisting disabled persons during emergencies.
Evacuation Procedures:
In the event of a partial or total evacuation due to fire or other emergency situation, the
following procedures will apply:
Begin the evacuation process upon hearing the general fire alarm or being given verbal
instructions to evacuate.
Stay calm.
Report anyone stranded to the Emergency Response Team member or Fire Department
personnel.
In the event you are unable to evacuate immediately press the Emergency button on the phone
and advise them of your situation and location. Telephones in public areas are No-Dial
Emergency phones.
Guest Room Safety/Security Features
All guest room doors are equipped with:
Peephole viewers.
All guest room connecting doors are equipped with 1-inch deadbolt lock.
Strict guest room key issuance policy in effect including photo ID upon check-in.
All guest rooms are equipped with smoke detectors and sprinklers.
Accessible guestrooms are fitted with emergency call devices linked to a 24 hour, 7 days
a week manned Security Control Center.
Security
The Hotel employees a uniformed, full-service security department 24hrs day / 7 days a
week.
Security staff members are trained in CPR, First Aid and use of AEDs.
The Security Team consists of trained male and female Security Officers.
Security is aided with the assistance of X-Ray equipment, Walk-through Metal Detectors
and hand-held detectors.
Secure basement vehicle park with valet-only parking and Security patrols.
Safe Deposit boxes are provided to all guests free of charge at our Front Desk. Available
24hrs day / 7 days a week.
Refresher training for all employees on safety and security issues is on-going.
Close liaison with local Police, CID and Emergency Services on regular basis.
All lost items found are handed to the Security officer at the Lost & Found office.
Any valuables found will be secured in our Lost & Found office and safes and are kept for
90 days.
The Hotel is not responsible for any items left in rooms, vehicles, or hotel premises.
Emergency Communication
Telephones: The campus telephone system will be used to the extent possible. In case
of system failure or a power failure, campus phones will not function. An alternative in
some buildings is the emergency single line phones, which could function in a power
outage. These phones, part of the ULSs Emergency Communications System (ECS).
Personnel will serve as messengers if phone communication is not an option.
Fire Alarm System: The building fire alarm system is continuously monitored for alarm
by a contracted service and, in a backup capacity, by the Shah Alam Police
Communication Center. All alarms result in an automatic response by Police station and
Fire Station.
They are aware that evacuation is required when the alarm system is activated.
