DEPENDENT
2013-2014 Verification Instructions

Your application was selected for review in a process called Verification. Verification is mandated by the U.S. Department
of Education and requires schools to gather additional documentation to check the accuracy of information submitted on the
FAFSA.
To facilitate this process, you are required to complete and submit a signed 2013-2014 Dependent Student Verification
Worksheet, (see below) and certain other forms of income documentation to the Financial Aid Office.
The income tax documents that we accept are based on policy put forth by the U.S. Department of Education. We can only
accept as documentation of tax information either:

A 2012 IRS Tax Return Transcript (we cannot accept a copy of your tax return)
OR
An updated FAFSA using the IRS Data Retrieval Tool

If you did not use the IRS Data Retrieval Tool when completing the FAFSA, we encourage you to go back to
www.fafsa.ed.gov and update your tax information via the Retrieval Tool option. Using this option is significantly faster
than requesting a tax return transcript from the IRS.
To Obtain a Tax Return Transcript from the IRS
If you are unable to use the IRS Data Retrieval Tool, please request a 2012 IRS Tax Return Transcript as quickly as
possible. There are a number of ways to request a transcript from the IRS:
1. Go to www.irs.gov
a. Under Tools, click on Order a Return or Account Transcript
b. Click the Order a Transcript in Section 3
c. Use the SSN, DOB, Street Address and Zip Code of the first person listed on the 2012 IRS Income Tax
Return (pay particular attention to the street address, using exactly what you used on the Return)
d. Click Continue
e. Select the Return Transcript for Tax Year 2012. Do not select the Account Transcript
2. Call the IRS at 1-800-908-9946
3. Complete IRS Form 4506T-EZ available at www.ecu.edu/financial. Go to Applying for Aid, then Forms &
Publications, then select 2013-2014 award year
** If the parents/marital partners filed separate 2012 IRS income tax returns, 2012 IRS Tax Return Transcripts must be
provided for each.
Individuals Who Were Victims of IRS Identity Theft
A victim of IRS identity theft who has been unable to obtain a 2012 IRS Tax Return Transcript or use the IRS DRT must
provide a signed copy of the 2012 paper IRS income tax return that was filed with the IRS and a signed copy of IRS Form
14039 Identity Theft Affidavit that was submitted to the IRS.
Individuals Who Filed Amended IRS Income Tax Return
If an individual filed an amended IRS income tax return for tax year 2012, provide both of the following:
1. A signed copy of the original 2012 IRS income tax return that was filed with the IRS or a 2012 IRS Tax Return
Transcript (signature not required) for the 2012 tax year; AND
2. A signed copy of the 2012 IRS Form 1040X, Amended U.S. Individual Income Tax Return, that was filed with the IRS.

Individuals Who Filed Non-IRS Income Tax Returns

If an individual filed or will file a 2012 income tax return with Puerto Rico, another U.S. territory (e.g., Guam, American
Samoa, the U.S. Virgin Islands, the Northern Marianas Islands), or with a foreign country, provide a signed copy of that
2012 income tax return(s).

Do not have transcripts sent directly to ECU Financial Aid. You must add the students name and ECU Banner ID before sending it
