Historical Perspectives on an Expanded Role for Nursing
Arlene W. Keeling, PhD, RN, FAAN
May 31, 2015 Summary: The article discerns and describes historical antecedents, foundation, documents, and legislative acts that served to set the stage for today, laying the preliminary for an expanded role for advanced practice nursing in the 21st century. Started in the year 1960, nurse practitioners referred to as a healthcare provider who provides a limitless, and extensive array of care. Lillian Wald and the Henry Street Settlement Visiting Nurses established Henry Street Settlement served different ranges of services to people. Demands for nurses' service increased, as well as the nurses on the HSS staff. nurses' dispensing practice (visits patients with physician-prescribed medicines) was questioned because nurses acted out things that are supposed to be done by the physicians at that time. Resulted to changes of boundaries with medical and nursing professionals Mary Breckenridge and the Frontier Nursing Service Certified nurse midwife founded Frontier Nursing Service travel with horses set up an advisory committee of physicians- developed a set of Medical Routines (standing orders) to legally cover the nurses practice. Political Forces at Mid-Century 1955 the American Nurses Association developed a model definition of nursing emphasized the fact that nurses were neither to diagnose nor prescribe The Nurse Practitioner Loretta Ford and pediatrician Henry Silver envisioned a nursing role that could bridge the gap between health care needs of children and families ability to access and afford primary health care. Nurse practitioner certification project was introduced In early 1970s a committee to study the extended roles for nurses had a critical examination of the professions boundaries. They have concluded that extending the scope of the nurse's role was essential to providing equal access to healthcare for all Americans. Advised national certification for nurse practitioners, and had developed a model nurse practice law that could be applied throughout the nation. Based on my understanding, the bottom line is for modern nurses should abide historical circumstances to reinstate elements of past practices in todays nursing practice. Where innovations are welcome to meet the new trend in the new generation with the application of earlier practices.
