Instructions:
Fill the appraisal form carefully.
Section 1 of the form should be filled by the appraiser.
Objectives written by appraiser in Achievement of Objectives portion of Section 1 must
be precise and objective. And the objectives must be related to the core job performed
by appraisee.
Appraiser should mention strengths & weaknesses of appraisee. And they must not be
related to any parameter already mentioned in B & C portion of Section 1.
Arrange a formal meeting with appraisee before giving final comments. Give him you
feedback about evaluation and highlight improvement areas. Also discuss his/her
personal goals.

b) Communicate strengths & weaknesses of the appraisee.
c) Discuss about his personal goals and development plans.
d) Take his feedback on his assessment.
e) Plan for next year.
f) Conclude on positive note.
Assessment should be unbiased and fair.
After that HOD should give his remarks against every parameter.
After reviewing HOD will return the forms to HR department.
