AFFIDAVIT OF NON-EMPLOYMENT
I, _______________, 29 years old, single, Filipino citizen and a
resident of _______________ Surigao del Norte, after having duly
sworn to in accordance with law , hereby depose and say THAT:
1. To this date, I am not employed in any capacity at any
corporation, company, individual person or with the
government;

2. I am executing this Affidavit of Non-Employment to attest


the truth of the foregoing and to support my application for
Correction of Clerical Error for R.A.10172 (GENDER) in my
certificate of live birth before the Municipal local civil
registrar of Albor, Province of Surigao del Norte;

3. I am executing this Affidavit to attest to the truth of the


foregoing facts and to have my gender in the certificate of
live birth change from male to female and for any other
legal purpose that this affidavit may serve best.
