Description:
On 04 October on Sahara-1, after finishing the pressure test for the stand pipe and top drive, the crew
broke the drill pipe to TDS joint. Then they broke the x-overs connections. When rotating the x-overs,
the drill pipe and TDS unscrewed and the drill pipe joint fell down into the 42tube guide.
The crew had taken a short cut by breaking the drill pipe/TDS joint first
This was an unsafe act, no one intervened
The crew forgot to latch the elevator
The crew forgot to cover the rotary table
