AFFIDAVIT OF DISCREPANCY

I, FELICIA LAFUENTE ILUSTRACION , of legal age, Filipino, married


and a resident of Purok IV, Taytay, Palawan, after having been duly sworn to
in accordance with law, depose and state THAT:

1. The correct spelling of my name is FELICIA EDENDEN LAFUENTE, as


stated in the Certificate of Marriage Contract filed in the Office of the Local
Civil Registrar;

2. The spelling of my name appeared as FELICIA EDINDIN in my


Certificate of Live Birth issued by National Statistics Office;

3. The names FELICIA EDENDEN LAFUENTE and FELICIA EDINDIN


LAFUENTE ILUSTRACION refer to one and the same person;

4. That the said discrepancy is not intentional and there is no intention


to conceal my true identity.

4. I am executing this Affidavit to attest to the truth of the foregoing


facts and to use the same for whatever legal purpose it may serve.

IN WITNESS WHEREOF, I have hereunto set my hand this 15th day of


September 2016, at Puerto Princesa City, Palawan.
