Oh&S Ia Auditor Course Learning Activity: Ohsas 18001 Conformity

The operator of a new machine press was well trained, but the risk assessment for the machine was over 2 years old from the previous "more or less the same" machine. No documentation existed to verify the similarities between old and new machines. The OHS manager only reviewed risk assessments after major incidents or workplace changes. This situation could be a potential non-conformity to OHSAS 18001 because risk assessments should be reviewed when new equipment is introduced. Clause 4.3.1 requires identification of OHS hazards and assessment of risks, and corrective action would be to conduct a new risk assessment for the new machine press.

OHSAS 18001 CONFORMITY
Maximum Time: 10 minutes This is an individual exercise

The incident
During an audit of the Galvin (UK) site you interview the operator of a machine press. He appears to be well trained and
well equipped for the job. He tells you the machines are in good order and are actually quite new (less than 6 months old).
When you ask to see the risk assessment for the machine, however, you notice that the latest risk assessment is over 2
years old. You check this out with the OHS manager and he tells you that the previous machine was more or less the
same as the new one so there was no need to review or update the old risk assessment as it was still broadly relevant.
You ask to see specifications for the old and new machine, so you can check out the similarities, but no such specs are
held, so you cant evaluate how similar (or otherwise) the old and new machines actually are. The OHS manager also
comments that it is general practice only to review risk assessments after a major incident or a major work place change

Is this situation a potential non-conformity to OHSAS 18001?

If you think it could be, please explain why, identify an appropriate OHSAS 18001 clause, and suggest what sort of
corrective action would be acceptable in order to resolve the problem

If you think it is more likely to be quite acceptable, again explain your decision, but identify at least 3 things that you would
need to verify before you could decide conclusively

Review Methodology
When everyone has made a decision and drafted their answers, we will review the situation as a group and compare (or
contrast) our views.
