AFFIDAVIT OF CONSENT AND WAIVER

I, ________________, of legal age, Filipino citizen, married, and with


residence and postal address at _______________________, after having
been duly sworn to in accordance with law, hereby voluntarily depose
and say:

1. That I am a petitioner for the degrees of Freemasonry particularly


with Masonic Lodge No. _____ in Daet, Camarines Norte;

2. That I hereby consent to the taking or sampling of my hair, urine,


or blood and submit the same for drug screening as one of the
requirements for my said petition;

3. That I further consent to make the result of my drug test


available to the concerned officers of the Freemasonry Lodge No.
_____ for their determination of my qualification;

4. That I hereby release the drug testing center and the officers of
the said Lodge from any and all claims which I might otherwise
have due to such results being made available and for the same
purpose I do hereby waive any and all rights to file action in
connection therewith;

5. That I shall indemnify and hold the drug testing center, the
Freemasonry of Lodge No. ____, and all its officers and members
harmless from any and all damages, attorneys fees, and costs of
suit which they or any of them may suffer on incur jointly or
severally, owing to my undergoing such drug screening and the
release of the result thereof.

IN WITNESS WHEREOF, I have hereunto set my hand this
