CERTIFICATION

TO WHOM IT MAY CONCERN:

THIS IS TO CERTIFY that the following is an excerpt from the Police Blotter of this
Police Unit dated November 3, 2016, to wit:
2016-001, 03 November 16, 10:30PM

(Stabbing Incident)

On this given date and time one Darwin Bordamonte 22 years old, student, native of
Sarangani, presently residing of No. 56 Papaya St., Dadianggas South, General
Santos City, personaly appeared to this police station and report the stabbing incident
transpired on Daproza Ave. Dadianggas North at about 10:00pm wherein victim one
Diego U. Aurora 22 years old, single, student, presently residing of Lot 9 Balimbing St.
Dadianggas North General Santos City was stabbed by a man identified as one Jun
Paul Macasil, aka Ponggok, 22 years old, single, laborer, a native of Sarangani and
presently residing at Lot 7. Balagtas St., Dadianggas North, General Santos City.
According to the residence they heared the suspect saying GAGO KA! SINULOT MO
SYOTA KO! and after that he stabbed the victim.the suspect immediately catches by
the concern citizen. SPO1 Jerald Calunsag and SPO4 Sherwen Lipat were dispatched
at the said place of incident to arrest and conduct an ocular investigation.

Disposition: This case was filed in court with CC no. 2016-0056, Brach, RTC,
General Santos City.

Given this 4th day of November 2016 at General Santos City, Philippines.
