This is the last Will and testament made at .on this .day

Whereas the Testator is the sole lawful owner, lessee, allottee and in

influence of allurement from anyone whatsoever. While in possession
of sound health and a disposing mind.

witnesses who have attested this will in the presence of the testator.

WHEREAS

I. EXECUTOR: I appoint _________________ as Executor of this my Last


Will and Testament and provide if this Executor is unable or unwilling to
serve then I appoint _________________ as alternate Executor. My
Executor shall be authorised to carry out all provisions of this Will and
pay my just debts, obligations and funeral expenses. I further provide
my Executor shall not be required to post surety bond in this or any
other jurisdiction, and direct that no expert appraisal be made of my
estate unless required by law.

II. GUARDIAN:

In the event I shall die as the sole parent of minor children, then I
appoint _________________ Guardian of said minor children. If this named
Guardian is unable or unwilling to serve, then I appoint

III. MARITAL STATUS:

IV. SIMULTANEOUS DEATH OF SPOUSE:

simultaneously with me or there is no direct evidence to establish that

simultaneously, I direct that I shall be deemed to have predeceased my
_________________ [wife or husband], notwithstanding any provision of

construed on such presumption.

V. SIMULTANEOUS DEATH OF BENEFICIARY:

If any beneficiary of this Will, including any beneficiary of any trust

husband], shall die _________________ within 60 days of my death or
prior to the distribution of my estate, I hereby declare that I shall be
deemed to have survived such person.

VI. BEQUESTS:

The testator declares that this is his Last WILL and has been executed
by his own free will. Without any force, compulsion, coercion, fraud,
undue influence of allurement from anyone whatsoever. While in
possession of sound health and a disposing mind not being actuated by
any duress, menace, or mistake.

In witness whereof I, the testator has executed this Will on the day,
month and year above first mentioned, in presence of the following
witnesses who have attested this will in the presence of the testator.

to be my last will and testament and I do hereby revoke all my earlier
Wills, Codicils and Testamentary dispositions. At the time of execution
of this Will I am in good health, and possess a sound mind and do

undue influence upon me from any quarter and this declaration
represents my true and correct intention with respect to my property.
