Melinda Blanco provided care for three patients on July 12, 2017. Her first patient was a 96-year-old man admitted for a fall who had high blood pressure that was lowered with medication. Her second patient was a 21-year-old man admitted after a motorcycle accident who had his Foley catheter removed after voiding. Her third patient was a 15-year-old boy she had previously cared for following a dirt bike accident, who had progressed greatly and now expressed gratitude for her care.

Day # 4: July 12, 2017

My day started as any other day. I was assigned three patients. My first patient was 96-year-old patient
who had been admitted the day before because he had fallen from his wheelchair. This patient has a left
orbital fracture and a laceration on his left lower eyelid. He is AAOx1 to person. Patient has a history of
BPH, dementia and stroke. His GSW was 14. Patient has a 20G left AC running NS + KCL 20 mEq at 80
mL/hr. Patient also has an 18G heplock on his right forearm. Upon initial assessment, the patients blood
pressure was 218/94 at 0753. We administered 2 mL of Labetalol via IV push. I had to push it in slowly
within a 2-minute span. After 30 minutes, we reassessed his BP which had thankfully come down. At
0823 it was 116/62. Patient had an order for a CT scan that was pending in order for the patient to be
transferred to WW9. We accompanied the patient to his CT and then we were waiting for a bed to open
up to complete his transfer.

My second patient was a 21-year-old who was admitted following a motorcycle accident. He suffers
from a sacral fracture and splenic laceration. For the most part, he was self-sufficient. His wife was with
him aiding us in his care. Ibuprofen 600 mg and propranolol 10 mg were administered at 0831. At 1000
we had an order to discontinue his Foley catheter and patient voided 300 ml at 1020.

My third patient was a 15-year-old who I had previously taken care of. Its the same patient who I have
previously mentioned that had a dirt bike accident. I administered his 15mg of propranolol and his
Percocet via his NG Tube at 1300. In my few weeks on the floor, he has progressed so much. He is now
able to speak and expresses gratitude. This patient who had been aggressive and restrained my first few
times on the floor was now grabbing my hand and thanking me for my assistance. That is such an
amazing feeling. This is why I chose nursing as a career. He just reminded me what a fulfilling career this
will be.
