JOINT AFFIDAVIT OF TWO-DISINTERESTED PERSONS

We, ERNESTO MANUEL LONTOC and JEAN MARIE MANATAD LLEGUE, both of legal age,
Filipinos, and with residence and postal address at Masagana St. Palico 4, Imus City, Cavite, and 31
Saturn St. Camella Homes, Salinas 2, Bacoor City, Cavite, respectively, after being sworn in accordance
with the law hereby depose and say:

1. That we both personally know KINGSLEY R. DE LOS SANTOS and have been part of his
family affairs and gatherings;

2. That we have learned that in his Pag-ibig data records, his full name was entered erroneously
as KINGSLEY R. DELOS SANTOS instead of KINGSLEY R. DE LOS SANTOS;

3. That we hereby attest under oath that his correct name is KINGSLEY R. DE LOS SANTOS
instead of KINGSLEY R. DELOS SANTOS;

4. That we are not in any way related with the DE LOS SANTOS family, nor have we received any
amount of consideration in executing this affidavit;

5. That this affidavit was executed to attest the truth of the foregoing document and purposes
for whatever legal purposes it may serve.
