AFFIDAVIT OF UNDERTAKING

I, _____________, of legal age, Filipino, single/married and a


resident of _______________, after having been sworn on oath, do
hereby depose and state, that:

1. I am an Enumerator under the National Stock


Assessment Program (NSAP) of the Bureau of Fisheries
and Aquatic Resources (BFAR) Caraga Region with office
address at Brgy. Masao, Butuan City, Philippines;

2. Pursuant to my job as an Enumerator, I will be


awarded with a Tablet Computer worth PHp 12, 340.00 for
the purpose of data monitoring;

3. I take full responsibility on the custody and care of


the Tablet Computer and in case of damage I shall report
and surrender the same to the Bureau;

4. Furthermore, I shall hold myself liable for the


payment of the Tablet Computer in case of its loss and shall
surrender it upon the termination of my contract and/or
upon my resignation ;

5. I undertake to abide and conform to all the terms and


conditions of my contract and the laws or orders BFAR-
Caraga;

6. This affidavit is executed for whatever legal purpose


it may serve.
