CHECKLIST OF REQUIREMENTS OF SPECIAL LAND USE PERMIT (SLUP)

A privilege granted by the State to a person to occupy, possess and manage in consideration of
consideration of specified return, any public forest lands for a specific use or purpose
AREA: depends upon the area requirement of the project
DURATION: 3 yrs; to be converted to FLAg or FlagT should there be a need to extent its validity
Approving authority: Regional Director

1. Duly accomplished application form


For Association, Corporation or Partnership
a. Articles of Incorporation/partnership
b. Resolution of the corporate governing body designating the authorized representative of said
corporation, association or partnership to apply/sign documents for and in behalf of the company
2. Payment of application fee of P500.00 except for mining prospective permit P1.50/ha or a fraction
thereof not less than P600.00 (DAO No. 2004 16)
3. NCIP Clearance (Certificate of Pre- and Prior consent)
4. Map of the area applied for with technical description, coordinates (2) sets of longitude and tie point
from the nearest landmark. (Note: Inspection and Survey fees of P400.00/km. Plus the actual available
and most economical transport cost of the survey team from the official station site (DAO No. 2004
16) ).
5. Inspection report duly signed by the inspecting officer attested by the CENRO officer concerned
6. Indicative management Plan
7. Initial Environment Examination (IEE) as basis for issuance of ECC
8. BIR Certification on the zonal valuation of the nearest commercial zone of the Baranggay/Municipality
or Province whichever is higher.
9. Proof of financial capability to develop and manage the area applied for
10. LGU/s endorsement (Baranggay/City/Municipal/Province) Resolution
11. For areas covered by specific laws, clearance/ authorization from a governing body
12. Posting of Performance Bond twice the annual rental of users fee as the case maybe but not less than
P10,000.00. Provided, that 50% of the computed bond deposit shall be posted in Cash and a balance in
the form of surety bond with a duration of five (5) years renewable every five (5) years in case of lease
or management agreement or coterminous of the Flag.
13. SLUP Rental is based on the DAO 2004 16
1 ha or less P90.00
Over 1 ha -5 has -P180.00 per hectare or fraction thereof
Over 5 has P300.00 per ha or fraction thereof
a. Updating of Rental for SLUP it shall be based on the 3% assessed/re-assessed value of the
land and 1% appraised value of improvement every 5 years thereafter
NOTE: SLUP application shall be considered for short term project. However, it will be converted to FLAg upon
renewal.
