2 STATEMENT OF FACTS

The parties were married on LIST DATE and separated on LIST DATE. The minor children

of this marriage are NAME OF MINOR CHILD, born DATE OF BIRTH.

6 List all children of this marriage, if any, or relationship along with

their date of birth.

9 On LIST DATE the Court issued an order for PARTY ORDERED TO PAY SPOUSAL
10 SUPPORT to pay the sum of $LIST AMOUNT per month for spousal support, payable one-half on

the first and one-half on the fifteenth of each month commencing LIST DATE. See the spousal
12
support order on file.

MOVING PARTY contends that the Court should order a vocational evaluation pursuant to
14
15 Family Code 4331 to be performed by LIST NAME OF PROPOSED VOCATIONAL
16 EVALUATOR on the grounds that the OTHER PARTY has the ability to obtain employment based
17
upon their party's age, health, education, marketable skills, employment history, and the current
18
availability of employment opportunities yet has failed to seek available employment opportunities
19
that would allow them to maintain herself or himself at the marital standard of living. See the
20
21 attached declaration of _____________ and exhibits attached thereto and incorporated herein by
22 reference.
