Private Lessons Contract

Initial, sign, and bring to your first lesson

Tuition for the upcoming month is due on the day of the last lesson of the current month.
Payments can be made by cash or check. Payments submitted after the due date are subject to
a 5% late fee. Payments may be broken down into smaller amounts over the course of the
month if prior arrangements are made with the instructor. A summary of charges will be issued
mid-month, around the second or third lesson of the month depending on the number of weeks.

In addition to tuition, the student or parent is responsible for purchasing materials required for
the lessons. This could include method books, sheet music, competition fees, performance
attire, among others. Fees for these materials will be included in the monthly summary of
charges if paid for by the instructor. The student also has the option to purchase materials
themselves.

Cancellations Policy Initial here ______


If the student needs to cancel a lesson, the teacher should be notified as soon as possible
through email or phone call. Make up lessons for student cancellations will be scheduled if the
instructor is notified at least 24 hours in advance. If the student does not come to the lesson
and no notification is given, scheduling a make up lesson is not an option.
If the instructor needs to cancel a lesson due to illness, emergency, or travel, the student will be
notified in advance and a make up lesson will be scheduled. The instructor will notify the
student as soon as a conflict is known.
If a lesson is cancelled due to inclement weather, a make up lesson will be scheduled.
Lessons will not take place on holidays. These cancellations will not be factored into the
monthly summary of charges.
Withdrawal/Termination Initial here ______
At least one months notice is preferred for intent to withdraw from the studio and private
lessons. In the event of a withdrawal with less than two weeks notice, the students account will
be charged for two weeks of lessons from the notification date. Formal notification in writing
must be submitted to the instructor and be signed by both student and parent/guardian, or
student only if they are 18 or older. Failure to come to lessons is not an acceptable notice of
intent to withdraw. If the student fails to come to 4 consecutive lessons, the students
enrollment in the studio will be terminated and their lesson time slot will be opened to other
students.
