MEMORANDUM OF AGREEMENT

KNOW ALL MEN BY THESE PRESENTS:


This MEMORANDUM OF AGREEMENT is made and entered into by and
between:
UNIVERSITY OF LUZON, College of Business Administration, with principal
offices and postal address at Perez Blvd, Dagupan City, Pangasinan, a duly
recognized educational institution by the Commission on higher Education
(CHED), represented by Dr. Amalia G. Dela Cruz, Dean of College of Business
Administration, hereinafter referred to as the Higher education Institution
(HEI);
-and-
The Luzon Medical Center with business/office and address at Dagupan,
Pangasinan represented by Ms. Virginia Legazpi, Accounting Head;

-WITHNESSETH-

WHEREAS, the University of Luzon offers Bachelor of Science in Business


Administration Program that require the students to undergo on-the-job
training in public or private offices as a requirement for graduation in order to
expose them to actual work situation as a means to enhance their formal
training;
WHEREAS, Luzon Medical Center Dagupan, willingly accepts trainees officially
enrolled in the HEI subject to the terms and conditions provided in this
Memorandum of Agreement;
Now, THEREFORE, for and in consideration of the foregoing premises herein
contained, the AGENCY and the HEI do hereby agree and stipulated the
following:
The CA/I shall:
a. Accept trainees endorsed by the HEI to undergo 250 hours of on-the-
job training;
b. Assign personnel to take charge and supervise the trainees. It is
understood that trainees will be assigned. Required to perform duties
and functions along their duties and functions in the CA/I;
c. Accomplish the necessary form required by the HEI in connection
with the trainees on-the-job training;
d. Issue the certificates of completion and a corresponding performance
rating based on the evaluation tool given by the HEI to trainees who
will successfully finish their on-the-job training;
e. Enforce rules that will govern the conduct of the trainees while inside
and within the premises of the CA/I, however, such rules shall be
made known to the HEI and trainees for proper guidance;
f. Designate an OJT Coordinator who will work closely with the CA/I to
monitor the actual conduct of the on-the-job training;

The HEI shall:


a. Endorse trainees who will undergo 500 hours on-the-job training in
the CA/I;
b. Designate an OJT Coordinator who will work closely with the CA/I to
monitor the actual conduct of the on-the-job training;
c. Provide the CA/I whatever documents that are required as the
trainees their on-the-job training; and
d. Together with the trainees and their parents, renounce and waive any
claim against the CA/I for injury that trainees may sustain of suffer,
personal and pecuniary, in the performance of their duties/ functions
while under training.
The CA/I and the HEI further agree that:
a. There will be no employer-employee relationship between the CA/I
and the trainees;
b. The trainees will be personally responsible for any damaged to the
property or injury that may result to international or negligent acts
while doing their duties and functions;
c. The CA/I reserves the right to discontinue the OJT of the trainees
upon submission of a written notice to the HEI; and
d. The HEI may pull out any students from the CA/I upon submission of
the written notice.
This Agreement shall take effect immediately upon signing hereof and shall
continue to be enforced until such time that anyone if the parties withdraws
from this agreement upon 30 days prior notice to the other.
In WITNESS WHEREOF, the parties have hereunto affixed their respective
