authorize my son Venan Rey L. Navarra to apply for an Instalment plan P3,199 and allow him to sign any
documents regarding this request on my behalf. I have no objection on this authorization.

I apologize that I will not be able to present myself due to the reason that I am currently living in our
provincial house far from where my son is applying in for an Instalment plans.

Rest assured that I take full responsibility for this authorization.

Attested to this letter is a photocopy of my ID for identification purposes. I hope for your kind and
