Good day. I am KEREN JOANI L. MORALES, a graduate of Iligan City East National High
School – Sta. Filomena from batch 2012-2013. I am now studying at MSU-Iligan Institute of
Technology (MSU-IIT). And I am residing at Purok 4, Luinab, Iligan City.

I am currently processing my documents for my passport. May I request from your good office
for a certified copy, with dry seal signed by the principal, of the following:

1.) High School Diploma – Original copy needed


2.) Form 137a (Transcript of Records) – Original copy needed.

The above items have to be certified for authentication by the Department of Foreign Affairs.
Your immediate attention on this matter will highly be appreciated since I’ll need this right after I
