Caleb Figuera​ Behavior Plan

Caleb​ will have a daily behavior chart for each. The behavior chart is sectioned off into the 3 main
blocks of the day.

Caleb’s​ Behavior Chart

Morning Block Middle Block Afternoon


8:00 - 10:15 10:30 - 11:10 Block

During each block ​Caleb​ will have opportunities to demonstrate expected behaviors. If ​Caleb​ is able
to meet expected behaviors, he will receive a star in the box. If ​Caleb​ demonstrates unwanted
behaviors, tally marks will be placed inside the boxes. If he receives 3 tally marks in one block, he will
not receive a star during that block. View image below for an example:

Behavior Chart will be sent home daily in planner. Rewards and consequences will be given at home
accordingly per Grandma.

Does not go over 4 tally marks in any given box

3 Stars Treasure Chest at the end of the day


As well as No chart below 2 Stars for that week Lunch Bunch with a friend on Friday

Consequences:
