MOTION TO RELEASE AWARD

COMPLAINANT, through the PUBLIC ATTORNEY’S OFFICE, by the


undersigned counsel and unto this Honorable Office, most respectfully
submits this Motion to Release Award, and states:

That Complainant hereby requests that the award in the amount of


ONE HUNDRED FOURTEEN THOUSAND EIGHT HUNDRED FIFTY TWO
PESOS & 65/100 (P114, 852.65) Philippine Currency, representing
SEPARATION PAY, BACKWAGES, PROPORTIONATE 13TH MONTH PAY ON
BACKWAGES, UNPAID WAGES, 13TH MONTH PAY and ATTORNEY’S FEES in
the above-entitled case be released in favor of the complainant.

WHEREFORE, premises considered, it is most respectfully prayed of


this Honorable Office that the award in the above-entitled case be released
and withdrawn in favor of the complainant.

Other relief just and equitable under the premises are likewise prayed
for.
