Dear Ma’am/Sir,

I/We Hereby Authorize to act for and on our behalf for the replacement and to receive
our Globe Post-paid sim card with cell phone no. under Account Number: and
Account Name: to comply with the requirements of Globe Telecom Inc. relative to this
request. As proof of identity of our representative, attached is a copy of his valid ID
with photo and signature which you may refer to subject to his presentation of the
original ID for validation.

We are aware that the replacement of sim card we are requesting to be collected by our
authorized representative on our behalf is already active, and, as such, should there be
unauthorized transactions upon release of our sim card to our representative, we will
not hold Globe Telecom Inc liable for the said transaction/s. Thank you.
