Blank IPCR Form

This document contains an Individual Performance Commitment and Review (IPCR) form used by the Supreme Court of the Philippines to rate employee performance. The form includes sections for employees to list their major final outputs and targets for the rating period along with measures, budgets, accountable divisions, and actual accomplishments. Employees are then rated on a scale of 1 to 5 for each output based on quarterly, effectiveness, and timeliness.
