Dear Ms Wright

Ref: Ms Jane Simms

I am writing to refer Ms Jane Simms, a 46 year-old single female, who was under your care prior to this
admission with recently exacerbated multiple scleroses three weeks ago. Ms Simms is now medically stable
and will be discharged to home.

Ms Simms has made good progress in all areas. Prothiaden 150mg daily has been added to her regular
medication regime. The ulcer on her right buttock is healed and she has maintained a low calorie diet
successfully. Ms Simms also exercises regularly and has had occupational therapy during hospitalization.

As per your understanding, Ms Simms lives with one unmarried sister in a house and since she stopped
working she has become more depressed. Hence, it would be helpful if you could continue the care of Ms
Simms after she returns home. Firstly, could you please closely monitor her weight and medications,
especially the newly prescribed medication-Prothiaden? Secondly, please assist Ms Simms to continue the
exercise program and maintain her skin integrity. In addition, could you please encourage Ms Simms to get
involved in some activities and develop new interests? This may help reduce her depression.
