Most humbly, I Ku. Manjulata Nayak aged about 50yrs. D/o:-Late
Harihar Nayak of Vill:- Chandibrahmapur, Po:- Mohapur P.S. Ramachandi
Dt:-Puri would like to intimate you that my father was a primary school H.M
retired from service on Dtd. 31/07/1994 from Siaro Prathamika Bidyalaya,
Siaro, Puri Sadar, Puri. After my father’s death my mother namely
Nishamani Nayak had been getting the family pension till her expiry on Dtd.
28/01/2015.
I came to know that there is provision of the Govt. to provide
pension to the unmarried, dependant daughter after the expiry of the
pensioner(s).
Now, being the unmarried, physically handicapped and dependant
daughter of late Harihar Nayak, I am requesting to obtain the said facility.
I earnestly request you to kindly consider my matter and enable me to
avail the facility as soon as possible.
