Reevaluation Form

This document is an application form for students of Dibrugarh University to request re-evaluation or re-scrutiny of exam answer scripts within 10 days of exam results being published. It requires the student to provide their personal details, exam details, original marks obtained, and surrender their original mark sheet. The principal or head of the department must also recommend that the request is genuine. A maximum of 2 papers can be re-evaluated/re-scrutinied with fees of Rs. 300 per paper for re-evaluation and Rs. 150 per paper for re-scrutiny. The final marks after re-evaluation will be retained even if lower than the original marks.

INSTRUCTIONS

1. The candidate seeking Re-evaluation / Re-scrutiny of his/her answer script must apply within
10 days from the date of publications of the results. No application will be entertained after
the stipulated period mentioned above.

2. Re-evaluation / Re-scrutiny in a maximum of only 2 (two) papers will be allowed.

3. Fee for Re-evaluation is Rs. 300.00 (Rupees three hundred) only per paper.

4. Fee for Re-scrutiny is Rs. 150.00 ( Rupees one hundred fifty) only per paper.

5. The application for Re-evaluation / Re-scrutiny must be submitted through the Principal /
Head of the P.G. Department concerned along with the requisite Fees. No application will be
received from the candidates direct.

6. Marks awarded after Re- scrutiny shall be retained as the final marks even if such marks may
be lower than the marks originally awarded and a fresh mark-sheet shall be issued. No claim
whatsoever for retaining the original marks shall be entertained.

7. Fee may be paid either at the University Cash Counter or through Bank Draft drawn in favour
of the Registrar, Dibrugarh University, Payable at the state Bank of India, Dibrugarh
University Branch.
