Use: Adverbs of Frequency:

To indicate approximately how many times something happens. always, constantly, frequently, often,
normally, usually, sometimes,
occasionally, rarely, seldom, never.
Place of Adverbs:

Adverbial Expressions:
 before simple tenses consisting of one word:
From time to time, now and then,
 after auxiliary verbs and the verb “to be”;
several times, again and again, hardly
 after the first auxiliary in compound tenses;
ever ( = almost never).
 after modal verbs;
 in questions they usually come after the subject.
Examples:
