CREATING A SIMPLE PATIENT DATABASE RECORDS

Encode the following in MS Excel.


For sheet 1: rename sheet1 to Hospital Records

Instruction: Integrate hospital record and patient record in sheet 3 (All Data). No copying and pasting process should
be made in Excel. Use only “vlookup” or “hlookup” functions to automatically combine the data. Save as
<lastname>_exercise 3.

SUMMARIZING DATA AND DATA PRESENTATION PROCESS

Instruction: The integrated data shown in sheet “All Data” should summarize be summarized in tabular form; the
suggested report and information are:
1. Summary of range of age of patient
2. Summary of socio economic class
3. Summary of work environment
4. Summary of civil status
5. Summary of profession
6. Summary of occupation

Interpret results.

VISUALIZATION OF REPORT USING GRAPHS AND CHARTS

Instruction: The integrated data shown in sheet “All Data” will be the basis of information and data. In addition, a
separate graph should be created showing the trend of age with height and weight of the patient using the table
shown below. Present the table in visual form; in addition, identify which of the patients are overweight,
underweight, and having a normal weight using vlookup or hlookup functions in excel.
