Dear Sir,

I, Abesheil I. Casulla, a second-year student taking up Bachelor of Physical Education Major


in Sports and Wellness Management, humbly asks for your permission to let me enroll for
this 1st semester of school year 2018-2019. I am sorry for being unable to enroll at the
prescribed time of enrolment for my family is facing a problem regarding my sister. And as a
daughter to my parents, it is hard for me to see them carry a problem that they were not
involved in the first place. And as a child, I feel that I must share the burden and help them
figure out the right decisions to make. I lost focus and eagerness to study because of that.

I am fully aware that I was wrong for being an irresponsible student. I am willing to accept
consequences and take any means just to continue my studies.

Furthermore, I will assure you that this situation will not happen again and I will always abide
by the rules and the regulations of this school.

Please accept my humble apology and request. Thank you, Sir and God bless.
