Greetings!

I am formally writing this letter to inform you that I have not received any formal
endorsements from Ms. Alma N. Paragua, RN, since I became the supervisor of the Out-
Patient Department. I already asked her several times to submit an endorsement but to
no avail.

May I ask that Ms. Paragua be inform of her duties to submit necessary requirements as
required of her. Furthermore, that she has to submit not only the list of equipment and
instruments but to have a formal endorsement with me.
