AFFIDAVIT OF DISCREPANCY
I,FREDIRICK MONTANEZ GONZALES ,of legal age, Filipino, married, and
a resident of San Luis, Caquilingan, Cordon, Isabela Philippines, having been
duly sworn in accordance with law, hereby depose and state:

That in my Certificate of Live Birth as recorded in the Office of the Civil


Registrar of Palanan, Isabela,Philippines, my name was corre written as as
FREDIRICK MONTANEZ GONZALES , however, my School Credentials,
School ID, Voters Certification and Certificate of Marriage was
ERRONEOUSLY spelled as FREDERICK MONTANES GONZALES;

That my true middle name is "MONTANES"; as appearing in the same


Certificate of Live as the maiden name of my mother AND my first name is
“FREDIRICK” AS stated in my Certificate of Live Birth;

That in all pertinent documents which I had used, to include that of my


scholastic records I erroneously misspelled my first name and middle name;

That I am executing this Affidavit of Discrepancy to attest to the truth of the


foregoing facts and to explain the discrepancy in my Certificates of Live
Birth, as regards my true first name and middle name.
