IN RE: PETITION FOR COMMISION AS NOTARY PUBLIC

Good afternoon your honor, I’m Atty. _______________________ respectfully


appearing as the petitioner in this case and I am ready to present and
mark my documentary exhibits to establish the jurisdictional facts:

In compliance with the 2004 Rules on Notarial Practice, and to prove that
I am a duly registered member of the Integrated Bar of the Philippines in
good standing and that I have never been convicted in any court of any
crime involving moral turpitude your honor, may I move that my:

Certificate of Membership in the Philippine Bar issued by the Office of the


Bar Confidant (Annex A) be marked as Exhibit A;

Certificate of Good Standing from the Office of the Bar Confidant,


Clearances from Municipal Trial Court in Cities, Laoag City, Regional Trial
Court and National Bureau of Investigation (Annexes B, C and D
respectively) be marked as Exhibit B, C, and D respectively;

The Official Receipt issued by the Integrated Bar of the Philippines


(Annex E) be marked as Exhibit E, Professional Tax Receipt (Annex F) be
marked as Exhibit F, Community Tax Receipt (Annex G) be marked as
Exhibit G;

Certificate of No Pending Case (Annex H) issued by the IBP National be


marked as Exhibit H, Certificate of Good Moral Character issued by the
IBP Ilocos Norte Chapter (Annex I) be marked as Exhibit I, Certificate
issued by the IBP Ilocos Norte Chapter signed by two executive members
of the IBP (Annex J) be marked as Exhibit J;

Official Receipt for the filing fee issued by the Office of the Clerk of Court
(Annex K) be marked as Exhibit K;

Likewise your honor, as required by Rule 3 Section 2 of the Notarial


Rules, May I also pray that:

Three passport size colored photographs of the petitioner with signature


(Annex L) be marked as Exhibit L;
The three specimen signatures and the true impression of my notarial
seal (Annex M) be marked as Exhibit M;

In compliance with the provisions of Rule 3 Section 5(a) of the 2004 Rules
on Notarial Practice, the Notice of Hearing and Order of this Honorable
Court dated August 23, 2017 for the publication of the said notice be
marked as Exhibit N;

The __________________________________, 2018 issue of “Ilocos Sentinel”, a


newspaper of general circulation be marked as Exhibit N, while page ____
column ____ thereof where the notice of hearing for the petition was
published be marked as Exhibit N-1;

The Affidavit of Publication of the Publisher be marked as Exhibit O.

That would be all your honor.

Your honor may we pray that that the verified petition be read in open
court to determine whether there are any opposition to the instant
petition;

There being no opposition your honor may I formally offer this exhibits
to prove jurisdictional facts and that I possess all the qualifications and
none of the disqualifications as a Notary Public and that I have read and
understood and am willing to abide the 2004 Rules on Notarial Practice.
