AFFIDAVIT OF CONSENT AND SUPPORT

WE, PEDRO and MARIA, both of legal ages, married to each other, Filipino citizens
and with residence and postal address at No. ______________________________, after having been
sworn to in accordance with law, hereby depose and so state THAT –

1. WE are the biological parents of minor LIWANAG, who is currently 17 years old
having been born on September 30, 2001;

2. Our daughter Liwanag will travel to Singapore for a tour/visit accompanied by her
aunt EMMANUELA for about three (3) to five (5) days;

3. We cannot accompany our child due to our work and financial constraints;

4. We personally guarantee the financial support of the child in the course of her travel
and stay outside the Philippines and further guarantee that she will not in any
manner become a burden to the Philippine and the Singapore governments.

5. We are executing this affidavit in connection with our child’s application for a
passport at the Department of Foreign Affairs and travel clearance from the
Department of Social Welfare and Development for her travel abroad.
