GUIDELINES FOR FILLING UP EXAM APPLICATIONS: (KPG CANDIDATES)

Please find attached Exam related applications. One application is required for one method. If the
candidate is appearing for a method Level 2 & Level 3, single application will be enough for that method.

1. Form -F77 – Initial application form to be filled by candidate and:


a) Part 1 – Candidate’s personal details – Regarding “certificate holder number” asked therein,
please leave the box blank as we will provide the number. Similarly attach a copy of ID proof
that the candidate would mention in the box asked for.
b) Part 4 – Pre-certification training – As candidates have already taken training in NDT Intl,
they may write in the Box “training certificate copy attached”. Similarly, candidates are
requested to attach a copy of their PCN certificate which they finally received from BINDT
for a proof.
c) Part 7 (candidate’s pre-certification experience) is to be attested by supervisor (ASNT Level
3, who holds the certificate for the same method the candidate is appearing. A ASNT Level 3
in UT, can also attest for the candidate who is appearing for PAUT & TOFD). A copy of the
said level 3’s certificate is also to be attached. Candidate’s pre-certification experience
requirement is 12 months minimum for a Level 2 method.
d) Part 10 – to be attested by Employer

2. Form-F 57 – (External Experience Verification) To be attested with Stamp by Supervisor (OR)


Employer for candidate’s each method and it may be attached with above referred Form 77.
3. Form-F 83 - Form F 83 (vision test)- to be done by the optometrist - The vision requirement is:
ISHIHARA Plates - 17 Numbers minimum candidate should read. Near vision test requirement is N4.5
or Jaeger No.1.
4. Form-F 78 – is applicable for 3.8, 3.9 (supplementary addition) candidates and is to be attested
by Employer in Part 7.
5. Form-F74 – Wallet Card application Form. Regarding “candidate’s number”, please leave the
box blank. The rest may be filled by the candidate till the rectangular box. We shall fill the data
below the rectangular box. Similarly please note that whatever the name candidate writes
against the “Given Name Box”, the same name will only be printed on his Wallet Card.
Accordingly he may write there his name as he wants it to be on the Wallet Card. Also please
arrange to provide passport size photograph 2 nos of each candidate.
