AFFIDAVIT OF ACKNOWLEDGMENT
I, ___________, of legal age, married, and a resident of ________,
after having duly sworn to an oath hereby declares:

That I am the father of ___________ who was born on ___________


in _______;

That I and __________, the mother of ________, were not married at


the time that she was born;

That I was not able to acknowledge my child at the time that her birth was
registered in the Civil Registry, because I was informed at the hospital that it was
not allowed since I and _______ were not yet married;

That as a consequence thereto, I am executing this Affidavit for the


purpose of acknowledging __________, and to expressly give my consent that
she would use my family name, DUTERTE, which he was already using in all her
school records and diploma and licenses;

That I am further executing this Affidavit to attest to the truth of the


foregoing facts and for whatever legal purposes and intents this may serve.
