AFFIDAVIT OF OWNERSHIP

I, RASHALYN D. JASON, of legal age, Filipino, single and a resident of Brgy. Fatima,
Villaba, Leyte, Philippines, after having been duly sworn to in accordance with law, hereby
depose and state, that:

1. That I am the true and absolute owner and possessor of ONE (1) UNIT LG
AIRCONDITIONING with Serial No. S-NO404TAKK02469 and Model No.
441F0134;

2. That I intend to deliver the said personal property as a collateral to secure the
loan that I am applying for with Palompon Community Multi-Purpose
Cooperative Incorporated (PACCI);

3. The above-described personal property is in my possession and is free from


any lien or encumbrance;

4. That I hereby warrant title and ownership over the above-mentioned personal
property and I will defend the possession of the Mortgagee from eviction;

5. That I execute this Affidavit of Ownership to attest to the truth of the


aforementioned facts and in support of my application for a loan with
Palompon Community Multi-Purpose Cooperative Incorporated (PACCI) and
for any other legal purposes that this Affidavit could serve.

IN WITNESS WHEREOF, I have hereunto affixed my signature this 16th day of July
2018 at Villaba, Leyte, Philippines.
