Dear Ms. Moneva:

We are from the Academics Arm of University of San Jose - Recoletos Junior Philippine Institute
of Accountants (USJR-JPIA), which is an organization under the Accountancy and Finance
Department, composed of BSA and BSMA students. We are planning to have our Community
Outreach Program this coming December 19, 2018 at the Bagong Lipunan Elementary
School at 2:00 P.M. We intend to donate books and supplies that are useful and are
beneficial for the approximately 224 Grade VI Students in your school. The event shall
also be participated by our Academic Arm members.

In view thereof, we would like to ask your permission to allow us to conduct the said
activity on aforementioned date, time and venue. Your favorable approval to our request
will contribute greatly to the success of this activity. Rest assured that the organization
will be held responsible for the order and safety of the participants and for the entire
activity. We are hoping for your favorable response to this request. Should you have
