JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS


RE: ONE AND THE SAME PERSON

WE, NERISA E. REPONTE and AGRIPINA A. DACALOS, both of legal age, Filipinos and all
residents of Inayawan, Cebu City, after having been duly sworn to in accordance with law, do
hereby depose and state that:

1. We personally know Malvin Cabaluna De Guzman, who is our long time neighbor and
acquaintance at Inayawan, Cebu City, Cebu, Philippines, and the fact of her birth is
registered at the Local Civil Registrar of City Cebu;

2. We know for a fact that the person by the name of "MALVIN CABALUNA GUZMAN " as
appearing in some documents or specifically in Social Security System Records with the
name "MALVIN CABALUNA DE GUZMAN " as appearing in her Marriage Certificate and
SSS ID, refer to one and the same person and that her true and correct name is MALVIN
CABALUNA DE GUZMAN;

3. We are not in any way related to any of the foregoing person within the fourth civil
degree either by consanguinity or affinity, nor do we have any pecuniary interest in this
matter whatsoever;

4. To remove confusion as to the two names, we hereby declare that the name MALVIN
CABALUNA GUZMAN and MALVIN CABALUNA DE GUZMAN refer to one and the
same person whose registered name is MALVIN CABALUNA GUZMAN;

5. We are executing this affidavit to attest to the truthfulness of the foregoing statements
and to establish the real identity of MALVIN CABALUNA DE GUZMAN and for all legal
intents and purposes it may serve.

IN WITNESS WHEREOF, we have affixed our signature this 2ND day of March 2019 at Cebu City,
Cebu, Philippines.
