 Unit Endorsement
 Notarized Affidavit of Confirmation
 Original Copy of PSA- issued Certificate of No
Marriage (CENOMAR-CRS Form No. 4) with
official receipt
 Original Copy of PSA – issued Birth Certificate of
PNP Personnel with Official Receipt
 Original Copy of PSA – issued Birth Certificates of
Dependents with Official Receipt
 Photocopy of ID with 3 specimen Signatures and
2x2 picture with nametag of Dependents and PNP
Personnel
 Original Copy of PSA- issued Death Certificate of
Dependent with official receipt
