Greetings of peace and goodwill!

We, the fifth year BS Accountancy students of UM Tagum College enrolled in Synthesis 101,
will be conducting a study to a certain depressed community which aims to look for problems
that confront the locale and recommend valuable solutions to help improve the quality of life
of the residents.

Having done the initial inquiry to the barangay chairman, purok chairman and the residents
of Purok 9 of barangay San Miguel, Tagum City, we decided to create a study that will help
solve several predicaments of the purok specifically the problem in livelihood. Hence, we
would like to establish a consumer’s cooperative at the said purok as a central focus of our
study.

With this, we would like to request from your good office the checklist of requirements in
establishing a consumer’s cooperative to help us realize the very goal and objective of the
study.
