Requirements in the Processing of Equivalents Record Forms (ERFs)

________ 1. Endorsement of the School Principal


________ 2. Letter Request of the teacher for salary upgrade
________ 3. Three (3) copies of fully accomplished ERF
________ 4. One (1) original and one (1) duplicate copy of Official Transcript of Record with
S.O. Number for private schools, and Board Referendum/ Resolution number for
public schools. For teachers who are non- teacher education graduates, units
earned in education should be included in the Transcript of Records
________ 5. One (1) original and one (1) duplicate copy of Certification from the Registrar as
to the MA units/ degree earned by the teacher (if public school)
________ 6. One (1) original and one (1) duplicate copy of Sworn Statement stating that the
teacher actually earned the units indicated in the Transcript of Records (Signed
by the Mayor/Notary Public and with updated Community Tax Certificate No.)
________ 7. One (1) original and one (1) duplicate copy of Certification from the Commission
on Higher Education (CHED) as to the masteral units/ degree earned by the
teacher (if private school)
________ 8. One copy of MA Curriculum/checklist certified by the Registrar
________ 9. One (1) original and one (1) duplicate copy of comprehensive complete Service
Record showing inclusive dates of paid service and salary adjustments/increases
granted duly certified by the Schools Division Superintendent or any authorized
official to certify service records
________ 10. One (1) certified photocopy of previously approved Equivalent Record Form as
Teacher II, if any, or Certification of no approved ERF as Teacher II (for Teacher III)
________ 11. One (1) copy of Approved Permit to Study
________ 12. PBET/LET Rating
________ 13. Teacher’s License
________ 14. Marriage Contract (if maiden name is reflected in other records), three (3)
authenticated copies (female)
________ 15. Latest Appointment (3 authenticated copies)
________ 16. One (1) original and one (1) duplicate copy of Notarized List of Teachers being
supervised by the proposed appointee with their item numbers ( for HT II & III)
________ 17. One (1) copy of thesis book (MA Degree)
________ 18. Three (3) copies of duly signed Plantilla Allocation List
________ 19. One (1) copy of Personnel Service Itemization and Plantilla of Personnel (PSIPOP)
________ 20. One (1) copy of latest pay slip
________ 21. Copy of NOSI, if any

 Only Teachers with Very Satisfactory Performance rating should be processed for
upgrading. The IPCRF with the latest performance rating should be included on the ERF.

 The maximum study load of nine (9) units per semester/ trimester should be strictly
followed. Teachers who are candidates for graduation may be allowed to carry
from one (1) to three (3) units more than the nine (9) units authorized, provided that
these units are the last units these teachers need to enable them to graduate.

 To easily identify the different requests of school divisions, it is suggested that


white colored folders be used for ERFs, reclassification of positions, retirements,
and other requests.

Requirements in the Processing of Reclassification of Position

_______2. Letter Request of the applicant for salary upgrade
_______3. Ample justification(s) for the need for the reclassification of position
_______4. Latest Appointment
_______5. Updated Service Records
_______6. Original Transcript of Records with Special Order
_______7. One (1) original and one (1) duplicate copy of Certification from the
Commission on Higher Education (CHED) as to the Masteral units/ degree
earned by the teacher (if private school)
_______8. CS Eligibility (Rating)
_______9. Updated PRC License
_______10. Copy of the latest post-audited Personal Services Itemization and Plantilla
of Personnel (PSIPOP) where the Item is reflected
_______11. Computation/Matrix for the resulting salary increment – 4 copies
_______12. Rank list of all personnel who are qualified for the desired position, duly
signed by the Chairman and members of the ranking Committee and
attested by the Schools Division Superintendent
_______13. Waiver from concerned personnel(s) in the event that the candidate is
not the most ranking
_______14. Plantilla Allocation List – 4 copies
_______15. Certification of Relevant Training with certified list
_______16. Enrolment Data (Form 3) in the present school assignment, including the
cluster schools handled, if any
_______17. VS Performance Rating for the last 3 consecutive years or two (2)
Outstanding Performance from the effectivity of the Latest Appointment
_______18. CSC Form 212 (Personal Data Sheet)
_______19. District Data Bulletin
_______20. Copy of Class Program of the Candidate (MT only)
_______21. Certificates/Proofs of Outstanding Accomplishment
_______22. Equivalent Records Form (ERF) for HT positions/ Position Description Form
for Principal positions
_______23. NEAP Certification as to the result of NQEP taken and Basic Training
Course for School Heads attended
_______24. SBM Task Force’s certification as to the rating obtained in the internal and
external stakeholders assessment
_______25. Notarized List of Teachers under supervision with the identification of their
respective plantilla item number per latest post-audited PSIPOP
_______26. Copy of the latest post-audited PSIPOP wherein the names of the
teachers under supervision are reflected
_______27. Certification of non-availability of item.
_______28. Payslip
