CREATION OF THE SCHOOL CHECKING COMMITTEE (SCC)

As stipulated in Deped Order No. 11, s. 2018 Guidelines on the Preparation and
Checking of School Forms dated March 7, 2018, a checking committee shall be created at the
school level.

In order to facilitate the timely, consistent and quality checking of school forms, M. B.
Asistio Sr. High School hereby creates the School Screening Committee composed of the
following:

Chair: Dr. Aurelio G. Alfonso


(School Head)
Vice Chair for Enrollment Mrs. Helen Grace L. Cabalag
Counts and Learner Profile : (EBEIS/LIS In-charge)
Vice Chair for Curriculum and Mrs. Lota C. Ibasco
Assessment : (Registrar)
Sub-committees for each Grade Class Advisers and other
Level: Subject Teachers

The School Screening Committee (SCC) shall perform the ff. functions:

a. The SCC will conduct a pre-checking activity involving the thorough review of all learners’
records for all classes in preparation for the scheduled checking of forms by the Division
Checking Committee (DCC).
b. The Vice-Chair for Enrollment Count will focus on LIS-Related Issues and school report
generation while the Vice-Chair for Curriculum and Assessment will ensure that the
content of the school forms and reports are compliant with the policies related to enrollment
eligibility, curriculum, promotion and retention.
c. The subcommittees will review the accuracy of the learner’s profile and enrollment
eligibility through school-level checking of the School forms (SFs).
