Affidavit of Appointment

I, ____________________, of legal age, married, Filipino, and with


postal address at 50 Seddon St., Waikanae, Wellington, New Zealand,
after having duly sworn to in accordance with law, depose and state that:

1. I am the registered owner of (Name of Business) situated at


(business address) with DTI Permit No. ___________ issued on (Date
Issued);

2. I am appointing (Name), to handle the operation of the said


business;

3. However, she/he needs more knowledge in handling the


business and decided to study abroad with academic course of Business
Administration- Diploma at Canadore College, North Bay, Ontario,
Canada, and will take more or less two (2) years or until August 31, 2020;

4. Considering the foregoing, the appointment given will take


effect after she had finished studying as she returns back to the
Philippines;

4. I am executing this Affidavit to attest the veracity of my


foregoing statements and for whatever legal purpose it may serve.
